复发性翼状胬肉中MUC5AC的表达与泪膜稳定性的相关研究

摘　　要：目的:检测粘蛋白MUC5AC在复发性翼状胬肉组织中的表达水平,初步探讨粘蛋白MUC5AC与翼状胬肉术后泪膜稳定性的相关性。方法:应用SP免疫组织化学染色法检测5例正常结膜组织、6例原发性翼状胬肉组织及22例复发性翼状胬肉组织中粘蛋白MUC5AC的免疫学定位及表达,所有病例术前均检查泪膜破裂时间(BUT)、泪液分泌(Schirmer)试验,分析MUC5AC在复发性翼状胬肉中的表达与BUT、Schirmer实验的相关性。结果:正常结膜组织、原发性翼状胬肉组织及复发性翼状胬肉组织的Schirmer试验、BUT组间差异有统计学意义,P<0.05;MUC5AC在正常结膜组织、原发性翼状胬肉组织及复发性翼状胬肉组织的表达组间差异有统计学意义,P<0.05;复发性翼状胬肉组MUC5AC阳性细胞计数与正常结膜组比较差异有统计学意义,P<0.05;Spearman相关分析,MUC5AC的表达与BUT呈负相关;r=0.616。结论:粘蛋白MUC5AC在复发性翼状胬肉中高表达与翼状胬肉术后泪膜稳定性下降有关。Objective：To detect the expression of MUC5AC in human recurring pteryguim and to evaluate the corrrelation between expression of MUC5AC and tear film stablity after pteryguim operation.Methods：The immunohistochemical SP method was used to examine the expression of MUC5AC in 5 cases of normal conjunctiva,in 6 cases of primary pteryguim and in 22 cases of recurring pteryguim,each case was need to be examine BUT and Schimer Test before operation. To analysis the correlation of expression of MUC5AC,Schirmer test and BUT.ResultS：There was significant difference in the expression of BUT and Schimmer Test and MUC5AC in normal conjunctiva,in primary pteryguim and in recurring pteryguim,P〈0.05.There was significant difference in the expression of positive goblet cells between recurring pteryguim and normal conjunctiva,P〈0.05.Spearman correlation analysis,there exists the negative correlation between MUC5AC expression and BUT,r =0.616.Conclusion：The unbalance of MUC5AC in recurring pteryguim correlates with the breakdown in tear-film stability after pteryguim operation.
