胆道恶性肿瘤组织耐药相关蛋白P-gp和GST-π表达临床意义的探讨

摘　　要：目的:探讨耐药相关蛋白P-糖蛋白(P-gp)和谷胱甘肽S转移酶(GST-π)在胆道恶性肿瘤组织中的表达及其与临床病理参数的相关性。方法:采用免疫组化SP法检测60例胆道恶性肿瘤组织中P-gp和GST-π的表达。结果:60例胆道恶性肿瘤组织P-gp阳性表达率为88.3%,GST-π阳性表达率为93.3%。肿瘤高、中分化组织P-gp和GST-π阳性表达率分别为79.3%和86.2%,均低于低分化组织的96.8%和100.0%,P值均<0.05;局部浸润组织P-gp和GST-π阳性表达率分别为96.4%和100.0%,均高于无局部浸润组织的81.3%和87.5%,P值均<0.05;淋巴转移组织P-gp和GST-π阳性表达率分别为97.0%和100.0%,均高于无淋巴转移的77.8%和85.2%,P值均<0.05。结论:P-gp和GST-π在胆道恶性肿瘤组织中呈高表达,联合检测P-gp和GST-π对指导临床化疗药物、制定合理的个体化疗方案和判断预后有着重要的意义。OBJECTIVE：To study the expressions and clinical significance of P-gp and GST-π in malignant tumor of biliary tract,and explore the correlation between clinicopathological parameters.METHODS： The P-gp and GST-πexpression from 60 cases of malignant tumor of biliary tract tissues were detected by SP immunohistochemical method.RESUTLS： The positive rate of P-gp was 88.3%,and the positive rate of GST-π was 93.3%.The positive rate of P-gp in high and moderate differentiation group was 79.3%,the positive rate of GST-π was 86.2%,and they were significantly lower than poor differentiation group（96.8% and 100.0%,P0.05）.The positive rate of P-gp in local infiltration group was 96.4%,the positive rate of GST-π was 100.0%,and they were significantly higher than no invasion group（81.3% and 87.5%,P0.05）.The positive rate of P-gp in lymphatic metastasis group was 97.0%,the positive rate of GST-π was 100.0%,and they were significantly higher than no lymphatic metastasis group（77.8% and 85.2%,P0.05）.CONCLUSIONS： The P-gp and GST-π have high expression in tissues of malignant tumor of biliary tract.Combined detection of P-gp and GST-π has important significance for guiding clinical chemotherapy drugs,formulating reasonable programming of individual chemotherapy and making correct judgment of prognosis.
