阑尾黏液性囊性病变的超声表现

摘　　要：目的研究阑尾黏液性囊性病变超声表现,提高诊断准确性。方法回顾分析16例病理证实阑尾黏液性囊性病变或其腹腔假黏液瘤患者超声资料。结果阑尾单纯性黏液囊肿1例及囊腺瘤2例:肿块管状或类圆形;囊壁薄光滑;囊内点状均匀回声;1个肿块周围见无回声区;肿块未见血流信号。局限性阑尾黏液性囊腺癌7例:肿块椭圆形、类圆形及欠规则形;3个囊壁不均匀增厚;囊内呈无回声、点状低回声及条索状高回声区;2个周围见无回声区;1个囊壁见血流信号。阑尾黏液性囊腺癌伴腹盆腔假黏液瘤4例,阑尾囊腺癌术后腹盆腔假黏液瘤2例:腹盆腔见游离腹水及多个囊性肿块伴分隔,肿块形态不规则,5例内见血流信号,2例患者右下腹另见阑尾囊腺癌表现。10例患者超声提示阑尾黏液性囊性病变或腹腔假黏液瘤。结论彩色超声对阑尾黏液性囊性病变有一定特征性表现,对其良恶性有一定诊断价值,是阑尾黏液性囊性病变的首选检查方法。Objective To evaluate the sonograpic features of appendiceal mucoceles and improve diagnosis capability.Methods Sixteen cases of histologically proved appendiceal mucoceles or its pseudomyxoma peritonei were retrospectively reviewed.Results There were one case of appendiceal simple mucocele and two cases of cystadenoma.They manifested as round or tubular,thin wall,encapsulated cystic masses with homogeneous contents.Anechoic area was found outside the wall in one mass.Blood flow signal was not seen.There were seven cases of appendiceal cystadenocarcinomas.They appeared as oval,round and irregular.The walls were thickened inhomogeneously in three masses.Internal echogenicity was heterogeneous with anechoic,hypoechoic and hyperechoic areas.Anechoic areas were found outside the wall in two masses.Blood flow signal was seen in the wall in one mass.There were four cases of appendiceal mucinous cystadenocarcinomas with pseudomyxoma peritonei and two cases of pseudomyxoma peritonei after appendiceal cystadenocarcinomas operation.They manifested as ascites and irregular cystic masses with isolations within the peritoneal cavity with blood flow signals were seen in five cases.Appearance of appendiceal cystadenocarcinomas were seen in the right lower abdomen in two cases.Ten cases were diagnosised as appendiceal mucoceles or pseudomyxoma peritonei by ultrasound.Conclusion The sonographic appearances of appendiceal mucoceles are characteristic.Ultrasonography can help predict the malignancy of appendiceal mucoceles and it is the first choice for diagnosis.
