摘　　要：目的探讨青年与老年胃癌的临床病理特点及影响预后的因素。方法回顾性分析669例胃癌患者的临床资料,并根据年龄分为≤30岁组、31～70岁组和>70岁组。对3组患者的病理特征及淋巴结转移等方面进行比较,Kaplan-Meier法计算患者的生存率,Log-rank检验分析年龄与患者预后的关系。结果不同年龄胃癌组之间在组织分化程度、组织类型、浸润深度以及淋巴结转移均存在差异,≤30岁组具有分化程度差、恶性程度高、侵袭力强、进展快的特点,其总生存率及预后最差,其次为>70岁组,31～70岁组预后相对较好。结论年龄与胃癌的临床病理特征关系密切,不同年龄人群具有不同的发病特点。Objective To explore the clinical features and prognosis of gastric cancer patients with different age stages. MethodsA retrospective study was made in 669 cases of the patients with gastric cancer.According to age,they were classified into three groups：the young patients（≤30 years old）,the middle-aged patients（31-70 years old） and the elderly patients（over 70 years）.Clinical data of 3 groups were analyzed such as pathological types and lymph node metastasis.Survival rate was calculated by Kaplan-Meier method,and the relationship between age and prognosis was analyzed by Log-rank test. ResultsThe significant differences were observed in depth of invasion,differentiation,histology and lymphatic metastasis among the three groups.The young patients with gastric cancer showed the clinical features of poorly differentiated,highly malignant,strong invasion and rapid development.And this group suffered the poorest survival and the worst prognosis in our study,followed by the eldest ones（70 years）.The prognosis of middle-aged patients was good relatively. ConclusionThere is a close relationship between age and the clinical characteristics of gastric cancer in adult patients,which leads to different clinical features in different age groups.
