原发性肝细胞癌患者乙型肝炎病毒表面抗原与肝组织HBV cccDNA水平的相关性

摘　　要：目的检测乙型肝炎相关性原发性肝细胞癌患者(hepatocellular carcinoma,HCC)HBV tDNA、HBVcccDNA,并探讨其与乙型肝炎病毒表面抗原(HBsAg)定量的相互关系。方法应用实时荧光PCR方法对37例HCC患者癌组织和癌旁组织HBV tDNA、HBV cccDNA及血清HBV tDNA进行定量检测;采用雅培化学发光法进行HBsAg定量检测。结果癌组织HBV tDNA、HBV cccDNA与HBsAg定量无相关性(r=0.146,P=0.388;r=0.151,P=0.371);癌旁组织HBV tDNA、HBV cccDNA与HBsAg定量无相关性(r=0.323,P=0.051;r=0.186,P=0.271)。癌组织HBV tDNA、HBV cccDNA及HBV cccDNA占HBV tDNA比例与癌旁组织比较差异均无统计学意义(6.61 lg拷贝/μgvs6.26 lg拷贝/μg,P=0.067;0.96 lg拷贝/μgvs1.33lg拷贝/μg,P=0.438;14.23%vs19.65%,P=0.501);HBV复制指标间相关性弱,仅癌组织HBV cccDNA与外周血HBV tDNA存在相关性(r=0.382,P=0.020),癌旁组织HBV tDNA与HBV cccDNA存在相关性(r=0.327,P=0.048),癌与癌旁组织HBV tDNA存在相关性(r=0.665,P<0.001)。结论 HCC患者病毒复制水平减低,癌与癌旁组织病毒复制水平无明显差异,且与血清HBsAg水平亦无明显相关性。Objective To analyze the serum level of hepatitis B surface antigen（HBsAg） and determine the quantization of total HBV DNA（tDNA） and covalently closed circular DNA（cccDNA） in the biopsy hepatic tissue of patients with HBV-related hepatocellular carcinoma（HCC）.Methods Thirty-seven HCC patients were randomly selected for the study.A real-time PCR assay was used for the quantization of intrahepatic HBV tDNA and HBV cccDNA,as well as serum HBV tDNA.Serum HBsAg level was measured by Abbott chemiluminescence kit.Results The level of serum HBsAg correlated with neither the intrahepatic HBV cccDNA nor HBV tDNA in the tumor and non-tumor tissues of HCC patients.There were no significant differences of intrahepatic HBV tDNA,HBV cccDNA and the proportions of HBV cccDNA between the tumor and non-tumor tissues.However,the serum HBV DNA level correlated with the level of intrahepatic HBV cccDNA in tumor tissues（r=0.382,P=0.020）.The intrahepatic HBV tDNA correlated with HBV cccDNA in non-tumor tissues（r=0.327,P=0.048）.The intrahepatic HBV tDNA of tumor tissues correlated with that of non-tumor tissues（r=0.665,P0.001）.Conclusions There was no significant correlation between the levels of HBsAg and intrahepatic markers of HBV replication.The levels of HBV replication were low and similar between the tumor tissues and non-tumor tissues of HCC patients.
