C-erbB-2蛋白在胆囊良恶性病变中的表达及意义

摘　　要：目的探讨C－erbB－2表达在胆囊粘膜癌变中所起的作用及与胆囊腺癌生物学行为的关系。方法采用免疫组织化学方法对10例慢性胆囊炎、12例胆囊腺瘤、3例胆囊腺瘤癌变和40例胆囊腺癌标本进行研究。结果C－erbB－2在胆囊腺瘤组的阳性表达率明显高于慢性胆囊炎组（P＝0．04）；胆囊腺癌组的阳性表达率明显高于胆囊腺瘤组（P＜0．01）；C－erbB－2表达与胆囊腺癌的分化程度、浸润深度，淋巴结转移无关（P＞0．05）；有结石组C－erbB－2阳性表达率明显高于无结石组（P＜0．05）。结论C－erbB－2在胆囊腺癌发生过程中起一定作用；C－erbB－2表达不能作为判断胆囊腺癌恶性程度和预后的可靠指标；胆囊结石在C－erbB－2基因活化和过度表达过程中可能起一定作用；胆囊腺瘤具有癌变潜能。Objective To explore the role of C - erbB - 2 gene in the canceration of gallbladder carcinoma(GBC) & the relationship between the expression and the bioligical behavior of GBC. Methode Ten specimens of chronic cholecystitis(CC), 12 specimeas of cholecystic ademoma(CA), 3 spcimens of ademoma with malignant change(AMC), and 40 specimens of GBC were studied with immunohistochemical method. Results The positive rate of C - erbB - 2 expression in CA group was significantly higher than that in CC group(P=0.04), and the positive rate in GBC group was significantly higher than that in CA group(P<0.01). There was no relation betwween the expression of C - erbB - 2 & the differentiation, the depth of invasion, and the lymph nodule metastasis in GBC(P>0.05). The positive rates of C -erbB-2 in group with gallstone were significantly higher than these in the group without gallstone(P<0.05). Conclusions C - erbB - 2 play certain role in the occurrence of GBC. The detection of C - erbB - 2 have not helpful in determining the degree of malignancy and the prognosis of GBC. Gallstone may play certain role in the activation and the overexpression of C - erbB - 2 oncogene. Cholecystic adenoma has a malignant potency.
