70例急性早幼粒细胞白血病初诊检测结果分析

摘　　要：目的探讨急性早幼粒细胞白血病（APL）初诊时检测结果的特点。方法收集70例APL患者初诊时检测结果,应用SPSS13.0软件进行统计学分析。结果 70例患者中WBC〈4.0×109/L38例（54.3%）,中位数1.45×109/L;Hb〈99g/L51例（72.9%）,中位数69.0g/L;PLT7~50×109/L51例（72.9%）,中位数26.0×109/L;D-二聚体351~1000μg/L30例（42.9%）,中位数761μg/L;1001~3000μg/L22例（31.4%）,中位数1618ug/L;单核细胞比值0.11~0.857范围36例（51.4%）,中位数0.225。通过骨髓细胞检查,发现异常早幼粒细胞增生,其胞浆内充满紫红色颗粒及柴束样奥氏小体。59例患者中细胞免疫表型CD33阳性,CD13阳性分别有46例（各占78.0%）,MPO阳性有44例（74.6%）。结论 70例APL初诊患者的外周血中WBC、Hb、PLT偏低,单核细胞以增高为主,D-二聚体均增高。因此,对于初诊外周血细胞偏低的患者,须借助显微镜进行细胞形态和特征分析,才能准确发出报告。Objective To investigate the characteristics of the acute promyelocytic leukemia（APL） detection results in preliminary diagnosis.Methods We collected detection results of 70 APL patients and implemented SPSS13.0 statistical software for statistical analysis.Results Among 70 cases,WBC 4.0 ×109/L were 38 cases（54.3%）,their median value was at 1.45 ×109/L,Hb 99g/L were 51 cases（72.9%）,median was at 69.0g/L,PLT 7~50×109/L were 51 cases（72.8%）,median was at 26.0×109/L.D-dimer 351~1000 μg/L were 30 cases（42.9%）,median was at 761.0μg/L,D-dimer 1001~3000μg/L were 22 cases（31.4%）,median was at 1618.0μg/L.Monocyte ratio 0.11~0.857 were 36 cases（51.4%）,median was at 0.225.The results of bone marrow cells showed proliferation of abnormal promyelocytic while their cytoplasm are filled with amaranth granules and bavin-like Auer.Among the 59 cases,46 cases（78.0%） were cell immunophenotyping CD33＋ and CD13＋ and 44 cases（74.6%） were MPO＋.Conclusion The results from 70 APL cases showed that the WBC,Hb,PLT in peripheral blood were at low level,mononuclear cells were high,and D-dimer were also increased.Therefore,the cell morphology and characteristics of patients which preliminary detection showed the peripheral blood cells decreased should be detected and analysed with the aid of the microscope in order to get accurate report.
