腹腔镜辅助与开腹胃癌根治术围手术期疗效的对比研究

摘　　要：目的:对比分析腹腔镜辅助胃癌根治术与开腹胃癌根治术的术后早期疗效。方法:回顾分析2005年至2010年为21例患者行腹腔镜辅助胃癌根治术的临床资料(腹腔镜组,laparoscopic gastrectomy,LG组),以同期66例开腹胃癌根治术作为对照(开腹组,open gastrectomy,OG组);对比两组患者的术后疼痛时间、出血量、术后排气时间、术后进食流质半流质时间及短期并发症等指标。结果:两组人口统计学指标、肿瘤发生部位及生物学特性无明显差别。LG组术中出血量明显少于OG组,术后恢复肛门排气时间、进流质时间及进半流质时间明显早于OG组(P<0.05),术后住院时间明显缩短(P<0.05)。结论:腹腔镜辅助胃癌根治术安全可行,能满足肿瘤根治的严格要求,在术中失血、术后恢复饮食时间、住院时间、术后并发症等方面均优于开腹手术。Objective：To compare the short-effects of laparoscopic-assisted with open radical gastrectomy for gastric cancer.Methods：The clinical data of 21 cases of laparoscopic-assisted surgery（LG group） and 66 open surgery（OG group） for gastric cancer from 2005 to 2010 were analyzed retrospectively.Clinical parameters included the degree and time of the pain,blood loss,time for passage of flatus,fluid and semiliquid food intake,hospital stay and postoperative complications.Results：There was no significant difference between two groups in term of demographic data,bionomics and the location of the tumor.The mean blood loss,time for passage of flatus,liquid and semiliquid food intake and hospital stay in LG group were significantly less than OG group.Conclusions：Laparoscopic-assisted surgery for gastric cancer is relatively safe and effective with superior short term outcome to open surgery.
