早期乳腺癌患者行乳腺癌根治术和保乳术后放疗的生存比较

摘　　要：目的探讨乳腺癌根治术和保乳术后放疗对早期乳腺癌患者生存情况的影响。方法选择早期乳腺癌379例,分为根治术组(341例)和保乳术后放疗组(38例)。分别观察两组患者的Karnofsky评分和复发情况。结果随访5年时保乳术后放疗组患者Karnofsky评分显著高于根治术组(P<0.05);保乳术后放疗组患者1、3、5年时的Karnofsky评分高于0个月和6个月时(P<0.05);根治术组患者3年时的Karnofsky评分高于0个月和6个月时(P<0.05)。根治术组和保乳术后放疗组随访3年的复发率分别为6.7%和5.3%,差异无统计学意义(P>0.05);随访5年的复发率分别为13.8%和7.9%,差异有统计学意义(P<0.05)。结论对于早期乳腺癌患者,保乳术后放疗较根治术更能提高患者生活质量和降低其复发率。Objective To explore the influence of radical mastectomy or radiotherapy after breast conserving surgery for the quality of life of patients with early stage breast cancer.Methods Three hundred and seventy-nine patients with early stage breast cancer were divided into the two groups of the radical surgery group（341 cases） and the radiotherapy after breast conserving surgery group（38 cases）.The Karnofsky scores and the recurrence rates were respectively evaluated in the two groups.Results The 5-year Karnofsky scores in the radiotherapy after breast conserving surgery group were significantly higher than those of the radical surgery group（P0.05）.The 1-,3-,5-year Karnofsky scores were higher than those of 0 or 6 months after surgery in the radiotherapy after breast conserving surgery group（P0.05）.The 3-year Karnofsky scores were significantly higher than those of 0 or 6 months after surgery and before surgery in the radical surgery group（P0.05）.The 3-year recurrence rate was 6.7% in the radical surgery group,and 5.3% in the radiotherapy after breast conserving surgery group（P0.05）;the 5-year recurrence rate was 13.8% in the radical surgery group,and 7.9% in the radiotherapy after breast conserving surgery group（P0.05）.Conclusion For the patients with early stage breast cancer,the quality of life in the radiotherapy after breast conserving surgery group is better than that of the radical surgery group,the recurrence rate in the radiotherapy after breast conserving surgery group is lower than that of the radical surgery group.
