摘　　要：目的探讨64层螺旋CT三期动态增强在肝外胆管癌(EHCC)诊断中的价值。资料与方法经过ERCP活检和手术病理证实的肝外胆管癌45例,采用64层螺旋CT平扫和三期增强检查,全部病例行多平面重建(MPR)和曲面重建(CPR)。结果 EHCC在64层螺旋CT的动态增强主要征象有胆管壁增厚、结节及肿块,胆管癌强化高峰通常出现于注药后60s及180s。诊断EHCC的准确度为96.0%,灵敏度为97.8%,特异度为93.3%。阳性预测值为95.7%,阴性预测值为96.6%。结论 64层螺旋CT三期增强可以为EHCC提供较准确的诊断信息。Purpose To investigate the diagnostic value of 64-slice spiral CT tri-phase dynamic contrast enhancement scanning on extrahepatic cholangiocarcinoma（EHCC）.Materials and Methods Forty-five cases of EHCC,which were confirmed by ERCP biopsy and incision specimen,were examined by 64-slice spiral CT.Plain scan and tri-phase dynamic contrast enhancement scan were acquired.MPR and CPR were performed for disease display.Results The CT features of EHCC included bile duct wall thickening,nodules or mass lesion.The peak enhancement occurred from 60 seconds to 180 seconds after contrast injection.The accuracy,sensitivity,specificity,positive prediction value,negative prediction value were 96.0%,97.8%,93.3%,95.7% and 96.6% respectively.Conclusion 64-slice spiral CT tri-phase dynamic contrast enhancement is helpful for EHCC diagnosis.
