摘　　要：目的评价早期前列腺癌抗原(early prostate cancel antigen,EPCA)在前列腺腺癌活检标本病理诊断中的应用价值。方法观察EPCA在196例前列腺腺癌和相关疾病[良性前列腺增生(BPH)、前列腺上皮内肿瘤(PIN)、不典型小腺体增生(ASAP)、炎症性萎缩后增生(PIA)]前列腺标本中的免疫组织化学染色情况。结果在149例前列腺腺癌病例中,EPCA染色阳性率为(95.3%),染色情况与Gleason评分、患病年龄、血清总PSA值的关系无显著意义。125例(83.9%)癌周有PIN或PIA,EPCA染色几乎都是阳性。结论 EPCA是早期诊断前列腺腺癌的阳性标记物,具有较佳的敏感性和特异性。在进行前列腺腺癌及其相关疾病的组织病理学诊断及鉴别时,还应结合HE切片、PSA、CK3413E12和p63等免疫组织化学染色情况以及临床资料进行综合考虑。Objective To evaluate the application value of early prostate cancer antigen（EPCA） in pathological study of the biopsy specimens of prostate adenocarcinoma.Methods Observation was made to the immunohistochemical staining of EPCA in prostate specimens of 196 cases with prostate denocarcinoma or other diseases,such as benign prostatic hyperplasia（BPH）,prostatic intraepithelial neoplasia（PIN）,atypical small acinar proliferation（ASAP） and proliferative inflammatory atrophy（PIA）.Result In 149 cases with prostate denocarcinoma,the positive rate of EPCA staining was 95.3%,there existed no correlation between the staining and Gleason scores,the age of the occurrence and the total PSA value in serum;in 125 cases with PIN or PIA around the cancer,the EPCA staining was all positive.Conclusions EPCA is the positive marker in the early diagnosis of prostate adenocarcinoma with good sensitivity and specificity;in the histopathological diagnosis and identification of prostate adenocarcinom or other diseases,the immunohistochemical staining of HE slice,PSA CK1413E12,p63 and the clinical data should also be taken into consideration.
