摘  要:目的 探讨大肠癌组织胎盘型谷胱甘肽 S转移酶( G S T Pi) 表达与其化疗耐药性的关系。方法 对64 例经术前化疗的大肠癌病例化疗前后的标本分别作 G S T Pi 和核增殖抗原( P C N A) 单抗 L S A B法免疫组织化学染色,以及原位末端标记( T U N E L) 法染色,分析 G S T Pi 表达与大肠癌化疗前后核增殖抗原( P C N A) 计数值和凋亡指数变化的关系。 结果 64 例大肠癌组织呈 G S T Pi 免疫染色阳性42例(66 % ) 。 G S T Pi 阳性的病例化疗前后大肠癌组织 P C N A 计数值分别为78 ±7 和78 ±8 ,其凋亡指数分别为29 ±04 和29 ±04 ,差异均无显著意义( P> 005) ;但阴性组经化疗后 P C N A 计数值从78 ±8降至73 ±6 ,凋亡指数则从28 ±07 升至56 ±09 ,差异均有显著意义( P< 001) 。 结论  G S T Pi 在大肠癌组织中过量表达不仅可对抗化疗药物抑制癌细胞增殖的作用,尚可对抗其促进癌细胞凋亡的作用。我们认为 G S T Pi 过量表达与临床大肠癌化疗耐药性有关,可作其预测指标。Objective To explore the relationship between GSTPi (Glutathion S Transferase) expression in colorectal carcinomas and their response to chemotherapy. Methods We used LSAB immunohistochemical technique to study the expression of GSTPi in 64 cases of colorectal cancinomas undergoing chemotherapy. Meanwhile, by determination of PCNA (proliferative cell nuclear antigen) and Tunel (TdT mediated X dUT nick end labeling) staining, we detected the proliferative activity and apoptosis of cancer cells before and after chemotherapy, whose relationship to GSTPi expression was then investigated respectively. Results Colorectal cancinomas in 42 out of 64 cases (65 6%) were positively stained with GSTPi monoclonal antibodies. Changes of PCNA and apoptotic index of cancer cells were not obvious in GSTPi positive colorectal cancers after chemotherapy ( P >0 05). However, the PCNA in GSTPi negative ones decreased significantly after chemotherapy ( P <0 01), and their apoptotic index was greatly increased ( P <0 01). Conclusions Excessive expression of GSTPi in colorectal cancers can not only help to resist the proliferation inhibiting action of chemotherapy, but also antagonize its apoptosis enhancing effect. Hence, the authors came to the conclusion that overexpression of GSTPi was associated with multi drug resistance of colorectal cancer, which may act as a prognostic factor for chemotherapy resistance.
