硅油继发青光眼的发病相关因素及治疗的初步探讨

摘　　要：目的　探讨硅油继发青光眼的发病相关因素及治疗。方法　对行玻璃体切割、硅油注入术的 2 75例患者术后随访结果进行回顾性研究。结果　硅油继发青光眼的发病率为 11.3 %。随着硅油在眼内存留时间的延长 ,其发病率增高。硅油乳化眼的硅油继发青光眼的发病率较无硅油乳化眼高 ,有晶状体眼与无晶状体眼相比发病率无显著性差异。3 1例硅油继发青光眼患者经药物和 /或手术治疗眼压可有效控制。结论　硅油继发青光眼是硅油手术的常见并发症 ,其发生与硅油眼内存留时间及硅油乳化有关 ,经药物和Objective To discuss the correlative factor and management of silicone oil induced glaucoma. Methods The postoperative courses of 275 cases that had undergone pars plana vitrectomy and silicone oil injection were reviewed retrospectively. Results The morbidity of silicone oil induced glaucoma was 11.3%.The morbidity was higher if the duration of silicone oil in vitreous cavity was longer.There was no statistical difference in the morbidity between the phakic and the aphakic eyes.There was a statistically significant difference in the morbidity between the emulsified silicone oil cases and not emulsified cases.The intraocular pressure was controlled to normal in 9 of 31 cases with medical treatment,and in the other cases with surgical and medical treatment. Conclusion Silicone oil induced glaucoma was a common occurrence after intravitreal silicone oil injection.Emulsification of silicone oil and the duration of silicone oil in vitreous cavity may play important roles in the development of glaucoma.Intraocular pressure can be controlled with medical and/or surgical treatment.
