卵巢浆液性囊腺性肿瘤中胎盘生长因子的表达及其意义

摘　　要：目的探讨胎盘生长因子(placental growth factor,PLGF)在卵巢良性、交界性、恶性浆液性囊腺性肿瘤组织中的表达及其与肿瘤微血管密度(microvessel density,MVD)和微淋巴管密度(lymphatic vessel density,LVD)之间的关系和临床意义。方法采用免疫组织化学法和RT-PCR法分别检测16例正常卵巢组织,23例良性、18例交界性和65例恶性卵巢浆液性囊腺瘤组织中PLGF蛋白和mRNA水平的表达,CD34和D2-40相关免疫组织化学染色法检测MVD和LVD的水平,分析PLGF的表达与MVD和LVD之间的关系及其意义。结果正常卵巢组织中PLGF蛋白表达阳性率为18.75%,卵巢良性浆液性囊腺瘤组织中PLGF蛋白表达阳性率为21.74%,卵巢交界性浆液性囊腺性肿瘤组织中PLGF蛋白表达阳性率为33.33%,卵巢浆液性囊腺癌为53.85%;PLGF蛋白表达与病理分期显著相关(P<0.05);PLGF mRNA水平的表达与蛋白一致,且与MVD和LVD水平均呈正相关(P<0.01,P<0.05),其与MVD的相关性更为密切。结论 PLGF在卵巢良性、交界性浆液性囊腺瘤和卵巢浆液性囊腺癌组织中的表达水平与临床病理分期密切相关,且与肿瘤血管新生密切相关,可能成为卵巢浆液性囊腺癌期治疗的新靶点。Objective To study the expression of placental growth factor （PLGF） in ovarian cystadeno carcinoma, ovarian benign cystadenoma and ovarian bordenline serous neoplasm and explore the relationship between the expression of PLGF and microvessel density （ MVD ） , lymphatic vessels density （ LVD ） as well as its clinical significance. Methods Immunohistochemical staining and RT-PCR were used to detect the protein and mRNA expression of PLGF in 16 normal ovarian, 23 ovarian benign cystadenoma, 18 ovarian bordenline serous neoplasm and 65 ovarian cystadeno carcinoma. CD34 and D2-40 related immunohistochemical methods were used to determine the MVD and LVD level. The association of PLGF expression with MVD and LVD levels was analyzed. Results The positive rate of PLGF protein was 18.75% in normal ovarian, 21.74% in ovarian benign cystadenoma, 33.33% in ovarian bordenline serous neoplasm and 53.85% in ovarian cystadeno carcinoma tissues. Its expression was significantly related with the clinical staging （ P〈0.05 ） . The mRNA expression of PLGF was consistant with its protein expression. There was appositive correlation between expression of PLGF and MVD and LVD （P〈0.01, P〈0.05 ） . Conclusion Both mRNA and protein expression levels of PLGF in normal ovarian, ovarian benign cystadenoma, ovarian bordenline serous neoplasm and ovarian cystadeno carcinoma.were related to its clinical staging and pathologic staging. PLGF has the potential to become a new target for early thereapy for ovarian cystadeno carcinoma.
