胆结石患者肝脏TREM-1、TREM-2表达的研究

摘　　要：目的探讨胆结石患者肝脏髓样细胞触发受体-1、2(TREM-1、2)的表达。方法收集临床上诊断为胆结石且行腹腔镜胆囊切除术的患者40例,并检测血浆转氨酶(ALT、AST)、血糖、血脂的含量以及计算体重指数,在胆囊切除术中取0.5cm×0.5cm×0.3cm大小肝脏组织(患者已签知情同意书)做HE染色,观察肝脏的形态学变化,并用免疫组化的方法检测肝脏TREM-1、TREM-2的表达水平。结果 (1)脂肪肝组的患者体重均呈进行性升高,体重指数高于同期非脂肪肝组(P<0.05)(;2)脂肪肝组ALT与AST明显升高(P<0.05),非脂肪肝组明显低于同期脂肪肝组(P<0.05);血糖、甘油三酯水平脂肪肝组与同期非脂肪肝组相比较差异有统计学意义(P<0.05);胆固醇水平脂肪肝组与同期非脂肪肝组相比较差异无统计学意义(P>0.05);(3)免疫组化结果显示胆结石脂肪肝组TREM-1及TREM-2的表达水平与非脂肪肝组相比较差异有统计学意义(P<0.05);(4)TREM-1的表达水平随着脂肪肝的严重程度(轻、中、重)而增高,TREM-2的表达水平随着脂肪肝的严重程度(轻、中、重)而降低。结论胆结石患者中脂肪肝组肝脏TREM-1表达增强,TREM-2表达减弱;非脂肪肝组肝脏TREM-1表达减弱,TREM-2表达增强。Objective To explore the expression of myeloid cells triggers receptor-1, 2 （ TREM-1, 2 ） in liver with Gallstones patients. Methods Collectting 40 patients that were diagnosed gallstones and will do laparoscopic cholecystectomy and detectting the plasma aminotransferase （ ALT, AST ）, blood glucose and lipid levels and health indices. Take the size of 0.5cm × 0.5cm × 0.3cm liver in the cholecystectomy （ patient has signed informed consent ） to do HE staining to observe morphological changes in the liver, and use immunohistochernical method to detect the expression of TREM-1, TREM-2 in liver. Results （ 1 ） The weight of patients in fatty liver group showed a progressive increase, weight index are higher than the non-fatty liver group （ P 〈 0.05 ） ; （ 2 ） ALT and AST in fatty liver group was significantly higher （ P 〈 0.05 ）, non-fatty hver group was significantly lower than the same period of fatty hver group （ P〈0.05 ） ; blood sugar, triglyceride in fatty liver group compared with the same period non-fatty liver group was significant differences （ P 〈 0.05 ） ; Cholesterol levels in fatty liver group compared with the same period nonfatty liver group was no significant difference （ P 〉 0.05 ） ; （ 3 ） Immunohistochemistry showed gallstones fatty hver group TREM-1 and TREM-2 expression levels compared with non-fatty liver group was significantly（ P 〈 0.05 ） ; （ 4 ） TREM-1 levels with severity of fatty liver （ mild, moderate and severe ） and get higher, TREM-2 levels with the severity of fatty liver（ mild, moderate and severe ） and get lower. Conclusion The expression of TREM-1 in fatty liver group with gallstones was increased, TREM-2 was decreased ; the expression of TREM-1 in non-fatty liver group with gallstones was decreased, TREM-2 was increased.
