摘 要:目的探讨牙齿缺失与食管癌生存的关系。方法采用前瞻性随访的方法,对来自林县营养干预试验一般人群的2743名食管癌患者随访观察24年,基线调查内容包括一般人口学信息、吸烟、饮酒、身高、体重、牙齿缺失状况等信息,Cox回归分析牙齿缺失与食管癌生存的关系。结果 5年随访研究结果表明,调整可能的混杂因素后,牙齿缺失暴露与食管癌死亡风险有统计学关联,RR=1.11(95%CI:1.01,1.21)。随访满15年和24年的研究结果与5年随访研究结果相似。将牙齿缺失情况分为三组,Ⅰ级、Ⅱ级、Ⅲ级,进一步分析。5年、15年、24年随访的研究结果也相似,男性牙齿缺失Ⅲ级均与食管癌的死亡风险有关,RR值分别为1.21(95%CI:1.01,1.44)、1.18(95%CI:1.00,1.40)和1.26(95%CI:1.06,1.50)。而女性牙齿缺失Ⅱ级、Ⅲ级与食管癌的死亡风险无关。结论男性牙齿缺失严重者食管癌死亡风险可能增加,而女性牙齿缺失与食管癌的生存可能无关。Objective To examine the association between tooth loss and the risk of esophageal cancer mortality.Methods A prospective study was conducted with a 24-year follow-up of the Linxian Population General Cohort Study′s 2743 esophageal cancer cases.Via Cox Proportional hazards regression,the association between tooth loss and esophageal cancer mortality risk was examined.The baseline questionnaire included questions regarding demography,smoking,alcohol drinking,height,weight and tooth loss status,etc.Results In a 5-year follow up analysis,tooth loss was associated with a significantly elevated risk of esophageal cancer mortality 1.11(RR=1.11,95%CI: 1.01,1.21) after controlling for potential confounders.We found the similar results in 15-year and 24-year follow up analyses.In 5-year,15-year and 24-year follow up analyses,severe tooth loss(defined as losing more teeth than the median tooth loss,stratified by age) was associated with the increased risk of esophageal cancer mortality in men(RR=1.21,95%CI: 1.01-1.44,RR=1.18,95%CI: 1.00-1.40 and RR=1.26,95%CI: 1.06-1.50,respectively).For women,moderate and severe tooth loss was not statistically associated with the risk of esophageal cancer mortality.Conclusion In this cohort,severe tooth loss may significantly increase the risk of esophageal cancer mortality in men and may be not associated with esophageal cancer mortality in women.
