复方苦参注射液对肺癌化疗中减毒增效作用的临床观察

摘　　要：目的观察复方苦参注射液(岩舒)在肺癌化疗中的减毒增效作用。方法将64例明确诊断的肺癌患者随机分为两组,治疗组与对照组各32例,均采用相同的化疗方案(小细胞肺癌用EP方案:足叶乙甙+顺铂;非小细胞肺癌用GP方案:吉西他滨+顺铂)及西医对症处理措施,治疗组加用复方苦参注射液静脉点滴,治疗3周期。结果治疗组有效率(CR+PR)56.2%,对照组43.7%,两组比较无统计学意义(P>0.05);治疗组临床受益率(CBR)93.7%,对照组75.0%,两组比较具有统计学意义(P<0.05)。化疗不良反应比较:治疗组与对照组恶心呕吐发生率分别为75.0%和93.7%(P<0.05),便秘发生率分别为28.1%和53.1%(P<0.01),白细胞、血小板下降分别为18.3%和43.7%、15.6%和37.5%(P<0.05);两组治疗后Karnofsky评分改善率分别为50.0%和28.1%,好转稳定率分别为96.8%和79.1%,治疗组高于对照组(P<0.05)。结论复方苦参注射液(岩舒)可提高肺癌患者治疗的总体临床受益率,明显降低化疗的不良反应,提高生活质量。Objective To observe toxicity attenuation and synergistic effects of Kushen（Radix Sophorae Flaves Centis） Injection on lung cancer chemotherapy.Methods Sixty-four patients with diagnosis of lung cancer were divided into two groups： treatment group and control group of 32 cases each.Same chemotherapy regimen（small cell lung cancer used EP regimen etoposide＋ cisplatin;non-small cell lung cancer used GP program：gemcitabine and cisplatin）and symptomatic Western medicine treatment were used.The treatment group was given compound Kushen Injection iv gtt in addition.Three cycles of treatment were given.Results The short-term efficacy（CR＋ PR） was 56.2% in treatment group and 43.7% in the control group,there was no significant difference between the two groups（P〉0.05）.Clinical benefit rate（CBR）comparison：treatment group 93.7%,control group 75.0%,with statistically significant difference（P〈0.05）.Adverse reaction comparison： Incidence of nausea and vomiting in the treatment group and control group were 75.0% and 93.7%（P〈0.05）,respectively,constipation rates were 28.1% and 53.1%（P〈0.01）,significantly lower in the treatment group,leukopenia and thrombocytopenia were respectively 18.3% and 43.7%,15.6% and 37.5%,lower in treatment group than in control group（P〈0.05）;post treatment KPS score improvement rates were 50.0% and 28.1%,stable rates of improvement rate were 96.8% and 79.1%,higher in treatment group than in control group（P〈0.05）.Conclusion Compound Kushen Injection could raise the CBR and QOL,and reduce adverse reaction of chemotherapy in patients with lung cancer.
