摘　　要：目的:回顾分析Ⅲa期非小细胞肺癌患者上皮型钙黏附蛋白(E-cadherin)和β-连接素(β-catenin)表达情况,探讨其对患者预后的相关性。方法:选取2005—2006年75例IIIa期非小细胞肺癌患者肿瘤组织标本构建组织芯片,经免疫组织化学方法完成E-cad-herin和β-catenin染色,分析E-cadherin和β-catenin表达与患者临床、病理及预后的相关性。结果:73例患者中有22例患者术后3年存活,3年生存率为30.67%,其中3年无瘤生存率为29.33%。肿瘤组织E-cadherin表达全部位于细胞膜,其中微量表达18例,低表达26例,中表达23例,高表达8例;β-catenin细胞膜表达71例,细胞浆和核膜表达4例,其中微量表达27例,低表达26例,中表达19例,高表达3例。E-cadherin和β-catenin表达与肿瘤分化程度呈正相关,E-cadherin表达为患者术后3年无瘤生存率的独立预后因素,风险比(hazard ratio,HR)为0.48(P=0.015);β-catenin表达与患者术后3年无瘤生存率无显著相关。结论:E-cadherin和β-catenin表达与肿瘤分化程度呈正相关,E-cadherin表达量是IIIa期非小细胞肺癌患者术后3年无瘤生存率预后的独立预测因素。Objective：To investigate the expression of E-cadherin and β-catenin as prognostic factors in stage IIIa non-small-cell lung carcinoma（NSCLC）.Methods： Tumor samples were constructed by tissue microarray（TMA） and immunohistochemistry（IHC） to analyze the relationship between the patients＇clinical significance and the E-cadherin,β-catenin expression.Results： A total of 22 cases survived after 3 years＇ follow up,the over-all survival was 30.67% and the disease-free survival was 29.33%.The number of trace,low,intermediate,and high-level expression of E-cadherin and β-catenin were19,28,28,12 and 28,32,33,4,respectively.There was a positive correlation between E-cadherin and β-catenin（P〈0.001）.Dedifferentiation was associated with reduced expression of both E-cadherin and β-catenin（P=0.001,P〈0.001）.E-cadherin expression was an independent prognostic factor for disease-free survival the hazard ratio（HR） was 0.48（P=0.015）.Conclusions： Reduced expression of E-cadherin and β-catenin correlates with poor differentiation.E-cadherin is an independent prognostic factor for stage IIIa NSCLCs 3 years＇disease-free survival.
