左手控制技术在右半结肠癌D3手术中的应用价值

摘　　要：目的探讨左手控制技术在右半结肠癌手术中的临床应用价值。方法 153例右半结肠癌手术患者,随机分为两组,其中78例使用左手控制技术清扫第3站淋巴结(观察组),同期使用常规手术方法的75例患者(对照组),其中C2期患者观察组为18例,对照组为10例,比较两组手术时间,术中出血量,住院时间和淋巴结清扫数量。结果观察组较对照组手术时间(P<0.05)和术中出血量(P<0.01)明显减少,Dukes C2期患者第3站淋巴结清扫个数增加(P<0.05),第3站阳性淋巴结数增加(P<0.05),而术后并发症发生率和住院时间无差异(P>0.05)。结论应用左手控制技术进行右半结肠切除可显著缩短手术时间、减少出血量并提高手术根治性。Objective To investigate the clinical value of the left hand control technology in the operations of right hemi colonic cancer.Methods The clinical data of 153 patients were studied who had undergone the D3 right hemicolectomy from June 2007 to October 2010,including 78 cases were undergone the left hand control technology in operations（the study group）,and 75 cases were used the general methods（the control group）.Results Compared with the control group,the operation time（P0.05）and blood loss（P0.01） were significantly decreased,the total number and the positive number of the No.3 lymph node dissection of the Dukes C2 patients were significantly increased（P0.05） in the study group.While the incidence of the postoperative complications and the hospital stay were equivalent to the traditional operations（P0.05）.Conclusion The use of the left hand control technology in D3 right hemicolectomy can significantly shorten the operation time,reduce the bleeding volumes and improve the effect of radical excision.
