HP感染与胃癌及癌前病变组织中hTERT表达的关系

摘　　要：目的探讨胃癌及癌前病变中幽门螺杆菌(helicobacter pylori,HP)感染情况及与hTERT蛋白表达的相互关系。方法采用Warthin-Starry银染法、免疫组织化学SP法分别检测66例胃黏膜肠化生、36例异型增生、50例胃癌及10例正常胃黏膜HP感染及hTERT蛋白表达情况。结果胃黏膜肠化生、异型增生、胃癌中HP感染率分别为60.61%、66.67%、58.00%,均显著高于正常胃黏膜,差异有显著性(P<0.05)。胃黏膜肠化生、异型增生及胃癌中hTERT蛋白阳性表达率分别为68.18%、72.22%、78.00%,均显著高于正常胃黏膜(P<0.01)。93例HP感染阳性组hTERT蛋白表达阳性率为88.17%,59例HP感染阴性组hTERT蛋白表达阳性率为47.46%,两组比较差异有显著性(P<0.01)。结论 HP感染、hTERT蛋白与及胃癌的发生有关,HP引起hTERT蛋白表达水平的上升可能是HP感染致胃癌的另一分子机制。Purpose To investigate the infection of helicobacter pylori（HP）and expression of hTERT protein in gastric cancer and precancerous lesions.Methods HP infection was detected by Warthin-Starry method,the expression of hTERT protein were examined by immunohistochemical technique SP method.Results HP infection in intestinal metaplasia,epithelial dysplasia of stomach and gastric carcinomas were 60.61%,66.67% and 58.00%,which were significantly higher than those in normal gastritis mucosa（P0.05）.The positive expression rate of hTERT protein were 68.18%,72.22% and 78.00%,which were significantly higher than those in normal gastritis mucosa（P0.01）.The positive expression rate of hTERT protein in 93 cases of gastric mucosa lesion of positive HP infection（88.17%） was significantly higher than that in 59 negative cases（47.46%,P0.01）.Conclusions HP infection and hTERT protein expression are associated with gastric carcinomas;that HP infection can result in decreased expression of hTERT protein may be one of molecular mechanism by which HP infection leads to gastric carcinoma.
