摘　　要：目的探讨结肠镜引导下腹腔镜下治疗结肠息肉的安全性和可行性。方法 2008年4月~2009年10月32例基底直径〉1.0 cm结肠镜下无法切除的结肠息肉在结肠镜引导下行腹腔镜下切除,其中21例广基息肉在结肠镜指导下腹腔镜切除并缝合修补肠管;11例腹腔镜辅助下小切口切除病变肠段。结果 32例均顺利完成手术,无中转开腹。手术时间平均70 min（40~150 min）,肠功能恢复时间平均28 h（20~30 h）。32例术后随访平均12个月（8~14个月）,未见复发。结论在结肠镜引导下腹腔镜切除结肠镜切除困难的结肠息肉,定位准确,切除彻底,是治疗结肠息肉有效、安全可行的方法。Objective To explore the safety and feasibility of laparoscopy guided by colonoscope for patients with colonic polyps. Methods From April 2008 to October 2009,32 cases of colonic polyps（diameter of the base 1.0 cm） that could be not removed by colonoscopy received colonoscope-guided laparoscopy in our hospital.Of the patients,21 cases with large polyps with thick stalk of the sigmoid colon underwent post-laparoscopic colon repair,the other 11 cases underwent laparoscopic segmental colon resection through a small auxiliary incision. Results All the 32 cases completed the laparoscopy successfully without complications or conversion to open surgery.The mean operation time was 70 minutes（40-150 minutes）,and mean recovery time for gastrointestinal function was 28 hours（20-30 hours）.The patients were followed up for a mean of 12 months（8-14 months）,none of them had recurrence. Conclusion Colonoscope-guided laparoscopy is safe and effective for patients with colonoscopically unrespectable colonic polyps.
