摘  要:目的 探讨丘脑静脉性脑梗死的影像学表现,以提高该病的诊断准确率.方法 回顾性分析2例经临床证实的丘脑静脉性脑梗死患者的CT及MR表现,并复习相关文献.结果 丘脑静脉性脑梗死的CT平扫表现为双侧(或)单侧丘脑及毗邻结构肿胀,密度降低.MR平扫表现为T1WI等、稍低信号,边缘模糊,T2WI为高信号,其内夹杂斑片状低信号影.MR增强扫描强化或不强化.静脉性脑梗死的特征性表现是大脑深静脉、静脉窦血栓形成,CT平扫表现为大脑深静脉系统内高密度影,MR T1WI为高信号,T2WI为大脑深静脉流空信号消失,呈等、高信号.结论 丘脑静脉性脑梗死CT及MR上较具特征性表现.MR是该病最敏感的检查方法.Objective To discuss the imaging manifestations of thalamic venous infarction, so that to improve the knowledge of this disease. Methods CT and MR imaging features of thalamic venous infarction in 2 cases confirmed clinically were analyzed retrospectively with literature review. Results On CT images, the involved thalamus (unilateral/bilateral) and its adjacent structures were engorged and of low density. On MRI, the infracted lesions showed hypointensity on T1WI and heterogeneous hyperintensity on T2WI compared to neighboring brain parenchyma. The lesion showed less or no enhancement after gadolinium administration. The thrombus in the deep venous system was the major manifestation of thalamic venous infarction, which showed high density on CT and high signal intensity on T1 WI. Due to absent or equal flow void signal intensity in deep venous thrombosis, high signal intensity on T2WI were seen. Conclusion The thalamic venous infarctions account for only a small fraction of ischemic strokes and carry a poor prognosis. These infarcts rarely have a venous etiology. CT and MRI can demonstrate its characteristic manifestations.
