非小细胞肺癌组织ERCC1和RRM1表达及其临床预后的相关性研究

摘　　要：目的探讨DNA修复基因蛋白ERCC1和RRM1在非小细胞肺癌(NSLCL)中的表达与临床预后的关系。方法采用免疫组织化学Envision法检测91例NSLCL组织中ERCC1和RRM1的表达。结果在NSLCL组织中,ERCC1阳性表达率为45.05%(41/91),RRM1表达率为70.33%(64/91),与癌旁组织比较,差别有统计学意义(P<0.01)。ERCC1和RRM1的阳性表达以及表达强度与患者的年龄、性别、肿瘤大小、组织学类型、淋巴结转移和临床分期无明显相关性(P>0.05)。临床Ⅱ期以上的ERCC1阳性表达者的生存率明显低于阴性表达者(P=0.004);RRM1阳性表达者以及临床Ⅰ期中ERCC1阳性表达者的生存率虽然高于阴性表达者,但差别无统计学意义(P>0.05)。结论临床Ⅱ期以上NSLCL组织中ERCC1的表达是独立于临床病理以外的具有预后评估意义的指标。Objective To investigate the relationship between the expression of ERCC1 and RRM1 in non-small cell lung cancer（NSCLC） and clinical prognosis.Methods Expression levels of ERCC1 and RRM1 in 91 NSLCL cases were detected by the Envision method.Results The total positive expression rate for ERCC1 and RRM1 was 45.05%（41/91） and 70.33%（64/91）,respectively,which was significantly higher than that in adjacent normal lung tissues（P〈0.05）.The expression and expression levels of ERCC1 and RRM1 were not correlated with the age,sex,tumor size,histological type,histological grading,lymph node metastases and clinical stages（P〉0.05）.In clinical Stage I,the survival rate of patients with positive expression of ERCC1 and RRM1 was higher than that of those with negative expression.In the other stages,the survival rate of patients with positive expression of ERCC1 was lower than that of those with negative expression,but that of those with positive RRM1 expression was higher than that of those with negative RRM1 expression.Conclusion The expression of ERCC1 and RRM1 in NSCLC tissues is a prognostic index for individual chemotherapy,which is independent of clinical pathology.
