摘  要:目的:探讨Shh、Glil、Snail及E-cadherin在胃癌中的表达及意义。方法:采用免疫组化方法检测54例胃癌组织、癌旁组织及30例正常人胃黏膜组织中Shh、Glil、Snail及E-cadherin的表达情况,并与临床病理资料做对照分析。结果:胃癌组织中Shh、Glil、Snail及E-eadherin阳性表达率分别为68.52%、61.11%、77.78%、25.93%;癌旁组织27.78%、24.07%、40.74%、70.37%;正常胃粘膜组织36.67%、26.67%、30.00%、96.67%。四种蛋白在胃癌组织和癌旁组织中的表达差异均具有统计学意义(P<0.05),Shh、Glil及Snail在癌旁组织及正常胃黏膜组织中的表达差异均无统计学意义(P>0.05),E-cadherin有统计学意义(P<0.05);四种蛋白的表达均与胃癌的分化程度、淋巴结转移相关(P<0.05),其中Shh、Glil及Snail与胃癌浸润深度有关(P<0.05),而E-cadherin无关(P>0.05);四种蛋白的表达与性别、年龄、位置及远处转移均无相关性(P>0.05);Shh、Glil和Snail三者之间在胃癌中的表达呈正相关(P<0.05),而这三个蛋白表达与E-cadherin呈负相关(P<0.05)。结论:Hedgehog信号通路与上皮间质转换(epitheli-al-mesenchyrnal transition,EMT)分子在胃癌组织中皆异常表达,提示异常激活的Hedgehog信号通路可能引起EMT分子的异常表达。这在胃癌的发生发展中可能起到一定的作用。Objective: To investigate the expression of Shh, Glil, Snail and E-cadherin in gastric cancer ( GC ) and its significance. Methods: Immunohistochemistry was used to detect the expression of Shh, Glil, Snail and E-cadherin proteins in 54 cases of GC tissues, their adjacent tissues and 30 cases of normal human gastric mucosa tissues. The correlation of Shh, Glil, Snail and E-cadherin expression with the clinicopathologic features of GC was analyzed. Results: The positive rate of Shh, Glil, Snail and E-cadherin expression was 68.52%, 61.11%, 77.78% and 25.93% in GC tissues, 27.78%, 24.07%, 40.74% and 70.37% in the paraneoplastic tis sues, and 36.67%, 26.67%, 30% and 96.67% in the normal human gastric mucosa, with a statistical significance between GC tissues and paraneoplastic tissues ( P 〈 0.05 ). There was no statistical difference in the expression of Shh, Glil and Snail between paraneoplastic tissues and normal gastric mucosa ( P 〉 0.05 ), while the difference in the E-cadherin expression was statistically significant ( P 〈 0.05 ). The expression of these four proteins was correlated with tumor differentiation and lymph node metastasis ( P 〈 0.05 ). The expression of Shh, Glil and Snail was related to the depth of invasion, while the E-cadherin expression was not ( P 〉 0.05 ). The expression of the above four proteins was not correlated with sex, age, location or distant metastasis ( P 〉 0.05 ). The expression of Shh, Glil and Snail was positively correlated with each other in GC ( P 〈 0.05 ), but was negatively correlated with E-cadherin ( P 〈 0.05 ). Conclusion: The expression of molecules involved in Hedgehog signaling pathway and Epithelial- mesenchymal transition (EMT) is abnormal in GC. The abnormally activated Hedgehog signaling pathway may lead to the abnormal expression of EMT related molecules and this mechanism may play a certain role in the oncogenesis and progression of gastric cancer.
