高频彩色多普勒超声在甲状腺乳头状癌诊断中的应用

摘　　要：目的探讨高频彩色多普勒超声对甲状腺乳头状癌定性诊断的价值。方法对95例经术后病理证实的甲状腺乳头状癌的高频二维超声和彩色多普勒声像图进行回顾性分析。结果 63例以实性密集低回声为主,边界不规则,呈蟹足样或有明显毛刺状,可见有砂砾样钙化。32例以混合回声为主,可有液性或混合性暗区,暗区周边不光滑。95个病例中,术前二维超声和彩色多普勒超声诊断为甲状腺癌63例,结节性甲状腺肿20例,甲状腺腺瘤11例,弥漫性毒性甲状腺肿(甲状腺功能亢进)1例,术前超声诊断与病理符合率为66.3%。结论甲状腺癌超声表现具有一定的特异性,高频二维和彩色多普勒超声对甲状腺恶性肿瘤的定性诊断有较高的应用价值。Objective To investigate the clinical value of high frequency color Doppler ultrasound in the diagnosis of thyroid carcinoma.Methods Ultrasonography of 95 patients with papillary thyroid carcinoma,which were all pathologically confirmed,were retrospectively analyzed.Results Sixty-three cases（66.3%） showed with solid dense hypoecho and blur boundary,and with gravel calcification.Thirty-two cases（33.7%） showed with mixed echo,liquid or mixed darkspace were accompanied,and the ambitus of the darkspace was rough.Among the 95 cases,preoperative ultrasonography showed 63 cases of thyroid carcinoma,20 cases of nodular goiter,11 cases of thyroid adenoma and 1 case of toxic diffuse goiter.The coincidence rate between the preoperation ultrasonograpy and the pathology was 66.3%.Conclusion High frequency two-dimension and color Doppler ultrasonography is valuable in the diagnosis of thyroid carcinoma.
