压抑疗法与遮盖法治疗学龄儿童单眼弱视的比较

摘　　要：目的研究压抑疗法与遮盖法治疗学龄儿童单眼弱视的临床疗效,并对两种方法的立体视重建、依从性、并发症进行比较。方法选取86例7～10岁单眼弱视学龄儿童,配对随机分成2组,两组弱视眼均配戴适矫眼镜,压抑组优势眼施行完全压抑法,涂用10g/L阿托品眼膏,开始7d每晚1次,之后每周涂1次,并戴欠矫眼镜(+3.00D～+5.00D),使优势眼戴镜视力低于弱视眼;遮盖组优势眼每天遮盖6h。从疗效、Titmus立体视重建、依从性及并发症四方面对两组进行比较。结果压抑组与遮盖组中、重度及总有效率比较差异均具统计学意义(P<0.05)。两组的Titmus立体视重建、依从性差异有统计学意义(P<0.05)。结论 本研究显示在学龄儿童单眼弱视的治疗中压抑疗法的疗效优于遮盖法;在立体视重建、依从性方面压抑疗法优于遮盖法。Objective To compare the penalization and occlusion treatment to school - age children with one - eyed amblyopia in terms of efficacy, impact on stereopsis reconstruction, compliance and complications. Methods 86 children with one - eyed amblyopia were randomly divided into 2 groups. 43 cases received penalization treatment, who were given 1% atropine ointment every 7 days after 7 days of continued medication, and added ＋ 3.00D - ＋ 5.00D based on the original diopter of the sound eye to make its visual acuity lower than that of the amblyopic eye. The other half received occlusion treatment, during which corrected glasses were used for the amblyopic eye while the sound eye was covered for 6 hours each day. The recovery efficacy of visual and Titmus stereopsis, compliance and complications were compared between the two treatments at month 24. Results There was significant difference between the two groups （ P 〈 0.05 ） in the rate of effect, moderate and severe amblyopia. The effective rate of Titmus stereopsis was 67.4% and 51. 1%, respectively, and there was statistically significant difference between the two groups （ P 〈 0.05 ）. The compliance rate between the two groups also showed significant difference（ P 〈 0.05 ）. Conclusions Where the curative effect, stereopsis and compliance are concerned, penalization is superior to occlusion therapy for school - age children with one - eyed amblyopia.
