摘　　要：目的:分析CT引导下经皮肺穿刺活检并发症的影响因素及其预防和处理。方法:回顾性总结我院185例CT引导下经皮肺穿刺活检术的结果,对主要并发症气胸和出血的相关因素行多分类非条件Logistic回归分析。结果:185例均穿刺成功,主要并发症为气胸和出血,其发生率分别为21.62%、6.49%。单因素分析年龄、吸烟、穿刺次数、病灶大小、穿刺角度、穿刺时间、穿刺深度等因素对并发症均有影响(p<0.05),多因素非条件Logistic回归分析年龄≤60岁、穿刺次数为1,穿刺时间≤30分钟为保护因素,而年龄>60岁、穿刺次数为≥3、穿刺时间>45分钟为危险因素。结论:年龄、穿刺时间、次数及深度是影响CT引导下经皮肺穿刺活检术并发症发生率的主要因素,警惕严重并发症的发生及预防。Objective：To explore the influential factors of complications caused by CT-guided percutaneous puhnonary puncture biopsy and analyze the methods for prevention and treatment. Methods： CT-guided percutaneons transthoracic lung biopsy was performed in 185 patients. Multinomial logistic regression was used to analyze the main complications pneumothorax and the related factors with hemorrhage. Results： CT-guided percutaneous transthoracic needle biopsy was succeeded in 185 cases. The major complications associated with CT-guided needle biopsy included pneumothorax （21.62%） and hemorrhage （6.49%）. Single factor analysis showed that these factors including age, smoking, puncture numbers, size of lesions, puncture angle, time of operation, puncture position had difference to the complicalions. （p 〈 0.05 ） The moltivariate logistic regression analysis showed that no more than sixty in age, puncture only one time, puncture time no more than thirty minutes were the proteclive faetors. However, more than sixty in age, puncture more than and eqnal to three times, punclure time more than forty-five minutes were the risk factors. Conclusion： The main factors influencing complicalions caused by CT-guided perculaneons puhnonary puncture biopsy are age, puncture time and puncture numbers. Severe complications should be considered during the procedure.
