摘  要:目的:探讨原发性食管癌组织中结肠腺瘤性息肉(APC)基因启动子区5-′CpG岛甲基化的状况及其与临床病理特征之间的关系。方法:实时荧光甲基化特异性聚合酶链反应检测182例食管癌及癌旁组织中APC基因启动子区5-′CpG岛甲基化状况。单因素分析采用Kaplan-Meier法,多因素分析采用Cox比例风险模型。结果:在182例食管癌组织中,APC基因启动子区5-′CpG岛甲基化率为54.40%(99/182),在182例相对应的癌旁组织中,甲基化率为9.90%(18/182),差异有统计学意义,P<0.05。APC基因启动子区5-′CpG岛甲基化分别与淋巴结转移、肿瘤远处转移、临床分期及不良预后差异有统计学意义,χ2=76.669,P=0.000。单因素结果提示,远处转移、分期及APC基因启动子区5-′CpG岛甲基化与食管癌患者的生存期相关,P值均<0.05。Cox多因素分析提示,APC基因启动子区5-′CpG岛甲基化是独立的预后因素,P<0.05。结论:APC基因启动子区5-′CpG岛甲基化在食管癌的发生发展中起重要作用,APC基因可能是食管癌新的肿瘤分子标志。OBJECTIVE:To detect the methylation of 5′-CpG island locating in the promoter region of APC gene in esophageal carcinoma,and analyze the relationship between the methylation of 5′-CpG island in the promoter region of APC gene and clinical manifestation.METHODS: The methylation of 5′-CpG island in the promoter region of APC gene was detected by real-time methylationspecific PCR,qMSP in all esophageal carcinoma tissues and peri-carcinomatous tissues.Comparision between groups was performed by log-rank test.Multivariate analysis was carried out by Cox proportional hazard method.RESULTS: In the tissues of 182 patients with esophageal carcinoma,the rate of the methylation of 5′-CpG island in the promoter region of APC gene was 54.40%(99/182),but the rate of the peri-carcinomatous tissues was only 9.90%(18/182).There was a notable difference between two groups(P〈0.05).Obvious positive correlation was found between the methylation of 5′-CpG island in the promoter region of APC gene and lymph node metastasis,tumor metastasis,clinical stage,poor prognosis(χ^2=76.669,P=0.000).Univariate analysis indicated that tumor metastasis,neoplasm staging and the methylation of 5′-CpG island in the promoter region of APC gene influenced the survival of the patients with esophageal carcinoma(P〈0.05).Multivariate analysis showed that the methylation of 5′-CpG island in the promoter region of APC gene was a independent prognostic Factor of survival(P〈0.05).CONCLUSION: The methylation of 5′-CpG island in the promoter region of APC gene may be involved in carcinogenesis and development of esophageal carcinoma,and might be a new molecular markers.
