摘 要:目的比较生物胶原膜与钛膜在引导骨组织再生技术中的应用。方法 75例牙槽骨骨缺损的种植患者共植入82枚Xive螺纹根状种植体,同期采用植Bio-Oss小牛骨粉,盖BME-10X胶原膜或钛膜,进行引导骨再生,修复骨缺损。其中39例采用钛膜,43例采用BME-10X胶原膜;二期手术时观察膜下骨再生情况,评价成功率。结果钛膜组37枚种植体、胶原膜组42枚种植体成功地完成骨整合,并成功完成义齿修复;胶原膜组和钛膜组的暴露率分别是2.3%和17.9%;2枚种植体因钛膜暴露及感染失败,1枚种植体因生物胶原膜固定不充分、移位感染失败。结论生物胶原膜及钛膜皆能有效地屏蔽软组织,引导骨再生,重建牙槽骨外形;与钛膜相比,生物胶原膜更为简便易用。Aim To assess the outcome of guided bone regeneration with titanium membrane or collagen membrane in dental implants placement.Methods Guided bone regeneration procedures employing resorbable collagen membrane(BME-10X) and non-resorbable pure titanium membrane were used for repairing and augmentation of the resorbed alveolar ridge around endosseous implants in 75 cases of patients with 82 Xive implants inserted.Pure titanium membrane was used in 39 implants while BME-10X collagen membrane was used in 43 implants.Postoperatively,some of these sites became exposed through the course of healing,while others remained submerged.Exposure rate of the membrane was 2.3%(1/43) for collagen membranes and 17.9%(7/39) for the pure titanium membranes.In periodic follow-up,the bone regeneration was observed by successive clinical and X-ray examination.At stageⅡsurgery,the bone regeneration under the membrane was observed.Results Thirty-seven implants with pure titanium membranes and forty-two implants with collagen membrane(BME-10X) had ideal osseointegration at stageⅡsurgery and were restored successfully with superstructures,while two of htirty-nine implants failed because of the early exposure of the pure titaniun membranes.One of forty-three implants failed because of the removal of the collagen membrane.Conclusion Both collagen membranes and pure titaniun membranes can effectively block off soft tissue,guide bone regeneration and restore alveolar bone contour.Collagen membranes is simple and easy to apply clinically and has less premature exposure rate than that of the pure titanium membrane.
