周围型肺癌CT表现特点与CYFRA21-1滴度相关性研究

摘　　要：目的探讨周围型肺癌CT表现与血清细胞角蛋白19片段(CYFRA21-1)滴度的关系。资料与方法回顾性分析48例周围型肺癌CT表现与血清CYFRA21-1滴度资料,对比分析CT表现与血清CYFRA21-1滴度的关系。结果 28例瘤体直径≥3 cm肺癌血清CYFRA21-1滴度(7.24±3.50)ng/ml,20例瘤体直径<3 cm肺癌血清CYFRA21-1滴度(4.80±4.18)ng/ml,两组血清CYFRA21-1滴度差异有统计学意义(P<0.05)。4例伴有空洞肺癌血清CYFRA21-1滴度(8.39±4.14)ng/ml,44例无空洞肺癌血清CYFRA21-1滴度(6.13±4.00)ng/ml,两组血清CYFRA21-1滴度差异有统计学意义(P<0.001)。肺癌有无深"分叶征"、"胸膜凹陷征"、"毛刺征"、支气管气相、增强CT值≥20 HU、"空泡征"、"毛玻璃征"、肺门或纵隔淋巴结增大与其血清CYFRA21-1滴度差异无统计学意义(P>0.05)。结论肺癌瘤体大小和空洞与其血清CYFRA21-1滴度有一定关系,瘤体直径≥3 cm或伴有空洞的肺癌血清CYFRA21-1滴度较瘤体直径<3 cm或无空洞的肺癌高。Objective To discuss the correlation between CT features and serum concentration of CYFRA21-1 in peripheral lung cancer.Materials and Methods The CT features and serum CYFRA21-1 concentration of 48 cases with peripheral lung cancer confirmed by operation and pathology were analyzed retrospectively.Results The serum CYFRA21-1 concentration of lung cancer were（7.24±3.50） ng/ml（n=28,diameter≥3cm）,（4.80±4.18） ng/ml（n=20,diameter3cm,there was significant difference between 2 groups（diameter≥3cm and 3cm in serum CYFRA21-1 concentration of peripheral lung cancer（P0.05）.The serum CYFRA21-1 concentration of lung cancer were（8.39±4.14） ng/ml（n=4,with cavity）,（6.13±4.00） ng/ml（n=44,without cavity）,there was significant difference between 2 groups（with cavity and without cavity） in serum CYFRA21-1 concentration of peripheral lung cancer（P0.001）.There was no significant difference between peripheral lung cancer with deep lobulation sign,pleural indentation sign,spiculation sign,spinous protuberant sign,increased CT value≥20 HU,lymph node enlargement of hilar or mediastinal and serum CYFRA21-1 concentration.Conclusion There is correlation between the diameter,cavity and serum CYFRA21-1 concentration in peripheral lung cancer.The serum CYFRA21-1 concentration is higher in peripheral lung cancer with diameter more than 3 cm or cavity.
