肺原发性类癌的影像学分析

摘　　要：目的：分析肺类癌的影像学表现,旨在提高本病诊断的准确率。方法：收集我院2006年1月~2010年7月间经病理证实的肺内类癌9例,均经MSCT扫描,其中,7例完成了CT增强扫描,1例完成了MR增强扫描,2例仅完成了CT平扫。回顾性分析患者的临床、CT扫描资料。结果：本组9例,5例位于支气管内,4例位于肺野内。其中,6例表现为圆形或椭圆形结节,3例表现为不规则结节;8例表现为边界清楚光整,1例表现为边缘毛糙;7例病灶密度均匀,有血管集束征2例,有钙化点2例。CT或MR增强扫描7例,6例见明显强化效应,CT值增幅超过40HU以上,仅1例见轻度强化效应。本组病例均未见空洞及胸腔积液。结论：肺类癌的常见影像学征象为：支气管及肺内边界清楚的类圆形肿块,增强扫描可有明显强化;可伴有钙化点,但空洞等继发征象较罕见。Objective：The purpose of this study is to analyze imaging findings about primary pulmonary carcinoid,which could improved the accurate rate of diagnosis.Methods：A retrospective analysis was performed to assess 9 cases with pulmonary carcinoid,confirmed by operation and pathology and performed examination using MSCT,at our hospital from 2006 to 2010.Among these,7 cases with CT enhancement,1 case with MR enhancement,2 cases only with CT plain scan.We retrospectively analyzed clinical and CT data about every patients.Results：Of all 9 cases,5 cases within bronchus,4 cases within lung field.Solitary round or ellipse tumors were shown in 6 cases,and other three cases appeared irregular.Clear and smooth boundary tumors were shown in 8 cases,and other 1 case appeared crude boundary.7 cases appeared homogeneous density,2 cases vessel bundling,2 cases calcification.7 cases performed CT or MR contrast scanning,6 cases appeared significant enhancement,which showed amplification of CT values exceed 40HU,and only 1 case appeared light enhancement.All cases were no cavity and pleural effussion.Conclusion：The imaging findings of pulmonary carcinoid include bronchus and pulmonary ellipse tumor with significant enhancement on CT or MR contrast scan,with calcification at part cases,but,cavity and secondary change are rare.
