摘 要:目的:应用ROC曲线比较三维超声与二维超声对先天性双子宫畸形诊断价值的差异。方法:2010年6月~2010年12月对45例可疑双子宫畸形的患者先后进行二维超声与三维超声冠状面成像检查,计算二维超声各项主要观察指标的诊断价值,与宫腔镜/腹腔镜或子宫输卵管造影的最终结果对照,应用spss13.0统计软件分别绘制二维超声与三维超声冠状面成像对双子宫畸形检出的ROC曲线下面积,比较2种方法的诊断价值。结果:二维超声ROC曲线下面积0.712,三维超声冠状面成像ROC曲线下面积0.928,2种方法经Z检验,Z值=2.051,P=0.04,提示三维超声冠状面成像优于二维超声。结论:本组病例提示二维超声和三维超声冠状面成像均能对先天性双子宫畸形做出较为准确的判断,在诊断效能上三维超声冠状面成像优于二维超声。Objective:To compare the diagnostic efficiency of three-dimensional ultrasonography and two-dimensional ultrasound in diagnosing congenital didelphus uterus by receiver operative characteristic(ROC) curve.Methods:Two-dimensional ultrasonography and three-dimensional ultrasonography multi-plane imaging were performed in 45 cases of dubious congenital didelphus uterus.Various given indicators were observed.The results were compared with hysteroscopy/celioscopy or hysterosalpingography.The areas under ROC curve of the two kinds ultrasonic imaging were compared using SPSS 13.0 statistic software.Results:The areas under ROC curve of two-dimensional ultrasound and three-dimensional ultrasonography with coronal plane were separately 0.712 and 0.928.Z value was equal to 2.051 through Z test and P=0.04.It showed that three-dimensional ultrasonography may precede two-dimensional ultrasound.Conclusion:Three-dimensional ultrasonography and two-dimensional ultrasound can both make correct diagnosis of congenital didelphus uterus.Three-dimensional ultrasonography coronal plane imaging is more reliable than two-dimensional ultrasonography in diagnosis of congenital didelphus uterus.
