香菇多糖注射液辅佐食管癌放疗的近期研究结果

摘　　要：目的观察免疫调节剂香菇多糖在食管癌放疗的近期增效、减毒、改善免疫功能的作用。方法以2009年1~12月住院治疗的60例Ⅲ期食管癌患者为研究对象,随机分为治疗组（放疗联合香菇多糖组,30例）和对照组（单纯放疗组,30例）,分别比较放疗前后免疫功能（T淋巴细胞亚群及NK细胞变化情况）、生活质量（卡氏评分）变化情况、近期有效率及血液系统毒性。结果治疗组治疗后CD3^＋、CD4^＋阳性率、CD4^＋/CD8^＋比率,以及NK细胞阳性率明显高于治疗前（P〈0.05）;CD8^＋阳性率治疗前后无显著性差异（P〉0.05）。而对照组治疗前后CD3^＋、CD4^＋、CD8^＋阳性率、CD4^＋/CD8^＋比率以及NK细胞阳性率均无明显改变。治疗组治疗后CD3^＋、CD4^＋阳性率、CD4^＋/CD8^＋比率,以及NK细胞阳性率明显高于对照组治疗后（P〈0.05）;CD8^＋阳性率无明显改变（P〉0.05）。治疗后生活质量改善情况治疗组优于对照组（P〈0.05）。治疗组总有效率优于对照组（P〈0.05）。治疗组患者白细胞及血小板下降较对照组为轻（P〈0.05）,两组治疗后红细胞及血红蛋白下降无差异（P〉0.05）。结论初步研究认为香菇多糖具有明显的减毒、增效、改善免疫功能及生活质量作用,并且安全、无明显毒性不良反应,可作为放疗的重要辅助用药。Objective To investigate the efficacy of radiotherapy combined with lentinan injection in treatment of esophageal carcinoma.Methods Sixty patients with esophageal carcinoma confirmed by histopathology were randomized into two groups： treatment group（30 cases getling radiotherapy combined with lentinan injection） and control group（30 cases getting radiotherapy alone）.Tlymphocyte and NK cells,treatment effectiveness,quality of life and hematologic toxicity were detected before and after the treatment.Results After treatment,CD3^＋,the CD4^＋ masculine gender rate,the CD4^＋/CD8^＋ ratio,and the NK cell masculine gender rate,the rate of KPS improvement,enhancement of treatment effectiveness,level of WBC and PLT of the treatment group were obviously superior to those of the control group（P〈0.05）.Conclusion In treatment of patients with esophageal carcinoma,lentinan injection with radiotherapy could improve the immunologic functions and the quality of life.
