干扰素合并氮烯咪胺治疗恶性黑色素瘤

摘　　要：目的 :评价大剂量干扰素α 2b(IFNα 2b)静脉滴注合并氮烯咪胺 (DTIC)方案治疗 12例恶性黑色素瘤的疗效、毒性反应和干扰素抗体的阳性率。方法 :用IFNα 2b( 18± 3 )MU/d ,DTIC 2 0 0mg/d ,iv ,5d/周 ,用 3周 ;第 4周用IFNα 2b 6MU ,im ,qd ,用 2周。用免疫酶标法 (ELISA)和抗病毒中和法分别检测患者血清中的干扰素结合和中和抗体。结果 :12例中 10例可评价近期疗效 ,获PR 4例 ( 4 0 % ) ,MR 2例 ( 2 0 % ) ,NC 3例 ( 3 0 % ) ,PD 1例( 10 % )。无复发生存期 >2年 1例 ,>1年 4例 ,>0 .5年 2例。 12例患者仅 1例结合抗体阳性 ,余结合及所有中和抗体均阴性。本方案毒性反应主要为骨髓抑制、发热、胃肠道反应、肝功能受损和神经系统毒性 ,Ⅲ级为 66.67% ,Ⅳ级为 2 5 %。结论 :IFNα 2b加DTIC方案治疗晚期恶性黑色素瘤近期疗效 。OBJECTIVE:To study the efficacy,toxic reactions and positive rate of interferon antibody to regimen of interferon alfa 2b and decarbazine(DTIC) in treatment of malignant melanoma.METHODS:12 patients with malignant melanoma were treated with IFNα 2b(18±3)MU/d and DTIC 200 mg/d given by infusion,five times weekly for 3 weeks;then they were treated with once daily IFNα 2b 6MU given by im route for another 2 weeks.The binding and neutralizing antibodies(BAb and NAb)were detected by EIA and anti virus neutralizing methods.RESULTS:The short time effect among 10 patients evaluated,4(40%)were PR,2(20%)were MR,3(30%)were N and 1(10%)was P and the relapse free survival time were :>2 years of 1 case,>1year of 4 cases,>1/2 year of 2 cases.Only 1 case of positive BAb was detected and the BAb and NAb in the rest of the 12 patients were negative.Myelosuppression,fever,GI reaction,hepatoxicity and neurotoxicity were the main side effects.CONCLUSION:The short time effect of advanced MM can be improved by regimen of IFNα 2b and DTIC and the survival time of these patients may be prolonged by this regimen in combination with local treatment.
