摘 要:目的:探讨急性中心性浆液性脉络膜视网膜病变(CSC)的形态学改变与视功能变化的相关性。方法:选取急性CSC患者36例(36只眼),应用光学相干断层扫描(OCT)和多焦视网膜电图(mfERG)进行检查。OCT测量CSC患者黄斑区神经上皮层脱离的高度、水平范围、黄斑体积。mfERG测量视网膜后极部不同区域的N1波、P1波的潜伏期和反应密度。采用Pearson相关分析和Spearman相关分析,分析急性CSC的黄斑区形态学改变与视功能变化的相互关系。结果:急性CSC患者黄斑区神经上皮层脱离高度、神经上皮层脱离水平范围与最佳矫正视力(BCVA)呈负相关(r分别为-0.922和-0.896,均P<0.001),黄斑体积与BCVA无相关(r=0.014,P>0.05)。mfERGP1波1~3环,N1波2环的潜伏期与BCVA呈负相关(rs分别为-0.488、-0.514、-0.452及-0.445,均P<0.05);N1波1、2环的反应密度与BC?VA呈负相关(rs分别为-0.497和-0.502,均P<0.05),N1波2环、P1波1环的潜伏期与神经上皮层脱离高度呈正相关(rs分别为0.486和0.421,均P<0.05)。结论:OCT和mfERG联合应用可准确检测CSC患者的黄斑区形态和功能的变化,CSC患者神经上皮层脱离的高度与黄斑区功能的变化关系密切。Objective:To investigate the correlation between macular morphology and functional characteristics in patients with acute central serous chorioretinopathy(CSC).Methods:Thirty-six patients(36 eyes) with acute CSC underwent optical coherence tomography(OCT) and multifocal electroretinography(mfERG) examinations.Heights,ranges of detachment in macular areas and macular volumes were mearsured with OCT.The latencies and amplitude densities of N1,P1 of retinal regions were mearsured with mfERG.Statistical analyses were performed by Person related analyses and Spearman related analyses,macular morphology and its correlation with macular function in patients with acute CSC were explored.Results:There was negative correlation between height,range of detachment in macular areas and the best visual acuity(BCVA)(the r values were-0.922 and-0.896,both P 0.001).But no correlation between macular volume and BCVA(the r value was 0.014,P 0.05).There was significantly negative correlation between BCVA and mfERG P1 latencies of ring 1-3,N1 latency of ring 2(the rs values were-0.488,-0.514,-0.452 and-0.445 respectively,all P 0.05).There was negative correlation between BCVA and mfERG N1 amplitude density of rings 1,2(the rs values were-0.497 and-0.502,P 0.05).There was negative correlation between height of detachment in macular area and P1 latency of ring 1,N1 latency of ring 2(the rs values were 0.486 and 0.421,both P 0.05).Conclusion:Combined mfERG and OCT can accurately detect the macular morphology and function in acute central serous chorioretinopathy.There was a close relationship between height of detachment in macular area and macular function.
