摘　　要：目的:探讨人附睾蛋白4(HE4)在子宫内膜癌组织及血清中的表达及临床意义。方法:选取2009年9月至20lO年3月宁夏医科大学附属31例子宫内膜癌患资料,用免疫组织化学法检测正常对照组(正常增生期子宫内膜)20例,子宫内膜增生组19例及子宫内膜癌组31例组织中HE4蛋白的表达水平,检测结果用image pro-plus 6.0专业图像分析软件进行定量分析,选择测定目标积分光密度(IOD)作为评价参数。应用酶联免疫吸附试验检测正常对照组(正常增生期子宫内膜)20例,子宫内膜增生组19例及子宫内膜癌组31例患者血清中HE4的水平。结果1)HE4在子宫内膜癌组的表达高于正常对照组及子宫内膜增生组,差异有统计学意义(P<0.001)。HE4在子宫内膜癌I期组织中的IOD中位数低于Ⅱ、Ⅲ期组织中IOD中位数,差异有统计学意义(P0.001)。HE4在子宫内膜癌组织中IOD与FIGO分期呈正相关关系(P<0.001)。2)子宫内膜癌组HE4的血清水平均高于正常对照组及子宫内膜增生组,差异有统计学意义(P<0.05);子宫内膜癌组术后血清中HE4的表达水平明显低于术前(P<0.05);3)子宫内膜癌组组织中的表达和术前血清中的水平呈正相关关系;4)子宫内膜癌组术前血清HE4水平与FIGO分期、血清CAl25呈正相关关系。结论:子宫内膜癌组织及血清HE4表达升高,可作为子宫内膜癌血清学标物,是子宫内膜癌的早期辅助诊断、疗效评价、预后评估的客观指标,可用于指导临床的综合治疗。Objective： To explore expression and significance of human epididymis protein 4 （ HE4 ） in tissue and serum of patients with endometrial cancer. Methods： Immunohistochemistry was used to detect the expression of HE4 protein in the tissue of the normal control （ NC ） group with normal proliferative endometrium （ n = 20 ）, the endometrial hyperplasia （ EnHy ） group （ n - 19 ） and endometrial carcinoma （ EnCa ） group （ n = 31 ）. Quantitative analysis of the detection was conducted by the pro-plus 6.0 profes- sional image analysis software, and the target integral optical density （ IOD ） was used for the judgement parameter. The enzyme-linked immuno-sorbent assay （ ELISA ） technique was applied to detect the HE4 level in the serum of the NC, EnHy and EnCa group. Results： The expression of HE4 protein was higher in the EnCa group than in the NC and EnHy group, with statistical differences among the three groups （ P 〈 0.001 ）. The median IOD of HE4 protein was lower in the group with stage I endometrial carcinoma than in the group with the stage- Ⅱ and Ⅲ carcinoma. The differences in positive IOD expression among the three groups were statistically significant （ P 〈 0.001 ）. The IOD of HE4 in endometrial carcinoma tissue was positively correlated with the FIGO staging （ P = 0.001 ）. The preoperative median level of HE4 was significantly higher in EnCa group than in the NC and EnHy group （ P 〈 0.05 ）. In EnCa group, the postoperative level of HE4 was significantly lower in the serum of EnCa group than the preoperative level （ P 〈 0.05 ）. In endometri- al cancer group, the expression of HE4 protein in tissue was positively correlated with the preoperative serum HE4 levels. The preoperative serum HE4 levels were positively correlated with the FIGO staging and the level of CA125 in the serum （ P 〈 0.05 ）.
