替吉奥单药口服治疗老年晚期胃癌的临床观察

摘　　要：目的观察口服替吉奥治疗老年晚期胃癌的近期临床疗效和不良反应。方法对45例有可测量病灶的老年晚期胃癌患者进行随机分组。替吉奥组22例采用替吉奥（S1）60 mg/m2.d,分早晚2次口服,第1~14天,休息7d为1个周期,连用3个周期评估疗效。对照组23例采用奥沙利铂（艾恒）130 mg/m2静脉点滴2 h第1天,四氢叶酸200mg/m2静脉点滴第1~5天,5氟尿嘧啶300 mg/m2静脉点滴第1~5天,3周为1个周期,3个周期后评估疗效。结果替吉奥组22例中CR 2例,PR 6例,客观有效率36.37%。对照组23例中CR 0例,PR 7例,客观有效率30.43%。2组有效率比较差异无统计学意义（P〉0.05）。替吉奥组和对照组便秘腹泻发生率分别为22.72%,56.52%,差异有统计学意义（P〈0.05）。周围神经毒性比较2组差异有统计学意义（P〈0.01）。2组病例Ⅲ~Ⅳ度不良反应发生率分别为4.55%和34.78%,差异有统计学意义（P〈0.05）。结论口服替吉奥治疗老年晚期胃癌的近期疗效肯定,较目前常规化疗安全、方便,患者耐受性好。Objective To evaluate the effect and toxicity of S-1 for elderly advanced gastric cancer.Methods 45 cases of elderly advanced gastric cancer were randomly divided into 2 groups：the S-1 group included 22 patients treated by twice daily of S-1 60 mg/m2·d,from day 1 to day 14,and repeated every 3 weeks.The control group included 23 cases treated by Oxaliplatin 130 mg/m2 in day 1,CF200 mg/m2 from day 1 to day 5,5-Fu 300 mg/m2 from d1 to d5,and repeated every 3 weeks.The effect and toxicities in the two groups were evaluated after 3 cycles of chemotherapy.Results In the S-1 group,there were 2 complete remissions（CR）,6 partial remissions（PR）,so the total response rate was 36.37%.In the control group,there were 0 complete remissions（CR）,7 partial remissions（PR）,and the total response rate was 30.43%.The difference of response in the two groups was not statistically significant（P0.05）.The incidence of diarrhea and constipation was 22.72% and 56.52% in the S-1 and control group,which had significant differences（P0.05）.There were also significant differences in the toxicities to the peripheral nerve and the rate of all Ⅲ~Ⅳ.adverse reactions（P0.05）.Conclusion S-1 is effective and tolerable in treatment of elderly patients with advanced gastric cancer.
