P-糖蛋白和细胞表面分化抗原在急性髓系白血病中的表达

摘　　要：目的分析初治急性髓系白血病(AML)P-糖蛋白(P-gp)及细胞表面分化抗原的表达。方法应用流式细胞术对81例初治AML患者进行P-gp及免疫表型的检测。结果 AML中P-gp的阳性表达率为39.5%,CD34为51.9%,Ly+AML为22.2%,其中CD7阳性率最高,为12.3%。P-gp与CD34均与预后为负相关(P<0.05),Ly+AML与Ly-AML的完全缓解(CR)率无统计学差异(P>0.05)。P-gp+组中CD34的表达显著高于P-gp-组(P<0.01),其它中晚期抗原表达无统计学差异。P-gp与CD34协同表达则CR率更低(26.1%vs 82.7%)。结论 P-gp的过度表达与CD34相关,P-gp+/CD34+的AML属于一种预后不良的临床亚型。Aim To explore the relationship and significance between P-glycoprotein（P-gp） and cell surface differentiation antigens in acute myeloid leukemiqa（AML） patients.Methods FCM was used to detect the expression of P-glycoprotein and immunophenotype in 81 de novo AML patients.Results The positive rates of P-gp,CD34 and Ly＋ in 81 AML paients were 39.5%,51.9% and 22.2% respectirely.Compared to other surface antigens,CD7（12.3%） was the common one in Ly＋ AML.P-gp and CD34 were negative correlation with prognosis in AML（P0.05）.The CR rates between Ly＋ AML and Ly-AML patients showed no significan differences（P0.05）.The positive rate of CD34 in P-gp＋ patients was higher than that of P-gp-patients（P0.01）.The CR rate in P-gp＋/CD34＋ patients was lower than that of P-gp-/CD34-patients（26.1% vs 82.7%）.Conclusions It is suggested that the expression of both P-gp and CD34 possesses prognostic significance in AML.The overexpression of P-gp is related to CD34 and AML patients with P-gp＋/CD34＋ predicts a poor prognosis.
