细胞染色体分析对良恶性浆膜腔积液鉴别诊断的临床研究

摘　　要：目的探讨细胞染色体检测分析对良恶性浆膜腔积液鉴别诊断的临床价值。方法染色体检测采取涂片染色镜检分析的方法。对伴有浆膜腔积液的43例恶性肿瘤和30例非恶性肿瘤的患者,抽取其浆膜腔积液进行染色体、癌细胞、CEA等检查。结果恶性染色体阳性结果的敏感性79.07%,准确性86.30%,明显优于癌细胞、CEA的检测阳性率,P〈0.05~0.01,且特异性高达96.67%。其中恶性肿瘤伴渗出性浆膜腔积液染色体阳性检出率90.32%;恶性肿瘤伴非渗出性浆膜腔积液阳性检出率50.00%,两组阳性率均较高,且P〈0.05。结论恶性肿瘤患者伴发浆膜腔积液时,无论是渗出液还是非渗出液,其染色体阳性检出率和特异性都很高,细胞染色体检测分析对良恶性浆膜腔积液的鉴别诊断具有很高临床适用价值。Aim To study the value of polyserositis with chromosome for differential diagnosis of benign or malignant Polyserositis.Methods The chromosome were measured with microscope after smear and stain.The chromosome,cancer cell,CEA were detected in all the 43 tumor patients and all the 30 no-tumor patients with polyserositis.Results The sensitivity of the chromosome detection was79.07%,the accuracy was 86.30% and the specificity was 96.67%,statistically significant superior than that of detection of cancer cell and CEA（P0.05～0.01）.Conclusions The results of chromosome detection showed that sensitivity and specificity in diagnosis of polyserositis by detection of chromosome are high,both in detection of percolate non-percolate of tumor patients.It is valuable for differential diagnosis of the benign or malignant polyserositis.
