摘 要:目的探讨64排CT全脑低采样灌注及血管成像在早期脑梗死中的应用价值。方法对2010年2月-2011年2月急诊科诊治的缺血性脑卒中患者36例采用64排CT(Philips Brilliance 64)行头部血管成像(CTA)及Jog模式CT灌注扫描。于工作站对图像数据行后处理,观察血管狭窄情况,比较各感兴趣区CBF、CBV及MTT。结果 (1)CTA共发现30例患者存在血管狭窄,占83.3%。(2)CT灌注图像中,脑梗死区及半暗带区与对侧正常对照区相比,CBF及MTT均有显著性改变(P<0.05)。脑梗死区CBV及CBF低于半暗带区,MTT长于半暗带区,其差异均有统计学意义(P<0.05)。结论 64排CT全脑低采样灌注及血管成像可以为早期脑梗死提供有价值的诊断信息,有利于患者及时、准确的治疗。Objective To investigate the application value of 64-slice CT perfusion imaging and angiography in the early stage of cerebral infarction.Methods 36 patients with ischemic stroke who were treated in the emergency department of our hospital from February 2010 to February 2011were selected including 21 males and 15 females,aged 39 to 86 years,mean 62.8 years old.Jog mode CT perfusion and CTA were performed using 64-slice CT(Philips Brilliance 64),and image data were processed in the Workstation,stenosis of the arteries were observed.The CBF,CBV and MTT values of each ROI were compared.Results 30 patients had artery stenosis in CTA images,accounting for 83.3%.In CT perfusion image,Compared with the contralateral control area,there were significant change the of CBF and MTT in the infarct area and penumbra.The CBV and CBF in the infarct area were lower than those in penumbra area,while the MTT were much longer.The differences were statistically significant(P0.05).Conclusion 64-slice CT perfusion imaging and angiography can provide valuable massage for the diagnosis of early cerebral infarction,it is good for timely and accurate treatment of the patients.
