摘 要:背景:结肠炎症是结肠肿瘤发生的危险因素,动物实验表明5-羟色胺(5-HT)与结肠炎症密切相关。结肠腺瘤患者结肠炎症与5-HT的关系尚未阐明。目的:探讨结肠腺瘤患者正常黏膜5-HT表达和炎症表现以及两者的相关性。方法:收集40例结肠腺瘤患者和17例对照组正常结肠黏膜。以免疫组化法检测上皮内淋巴细胞(IEL)和固有层CD3+T细胞、5-HT阳性肠嗜铬(EC)细胞、嗜铬素A(CgA)阳性EC细胞计数,实时荧光定量PCR法检测肿瘤坏死因子-α(TNF-α)、环氧合酶-2(COX-2)和诱生型一氧化氮合酶(iNOS)以及色氨酸羟化酶1(TpH1)mRNA表达。结果:结肠腺瘤患者正常黏膜固有层CD3+T细胞、5-HT阳性EC细胞、CgA阳性EC细胞计数较对照组显著增高(P<0.05),IEL计数与对照组相比无明显差异(P=0.175)。结肠腺瘤患者正常黏膜TNF-α、COX-2、iNOS、TpH1 mRNA表达较对照组显著增高(P<0.05),且TpH1 mRNA表达与TNF-α、COX-2、iNOS mRNA表达均呈正相关(P<0.05)。结论:结肠腺瘤患者正常黏膜存在炎症,肠道5-HT合成增加与腺瘤患者结肠黏膜炎症相关。Inflammation is a risk factor of colonic tumor. Animal studies suggest that 5-hydroxytryptamine (5-HT) is closely associated with colonic inflammation. The relationship between 5-HT and colonic inflammation in patients with colonic adenoma is unclear. Aims: To study the expression of 5-HT and inflammation in normal mucosa of patients with colonic adenoma and the relation between 5-HT and inflammation. Methods: Samples of normal colonic mucosa from 40 patients with colonic adenoma and 17 normal controls were collected. Immunohistoehemistry was used to assess cell counts of intraepithelial lymphocyte (IEL) and lamina propria CD3+T lymphocyte, 5-HT-positive enterochromaffin (EC) cell and chromogranin A (CgA)-positive EC cell. mRNA expressions of tumor necrosis factor (TNF)-a, cyclooxygenase (COX)-2, inducible nitric oxide synthase (iNOS) and tryptopban hydroxylase 1 (TpH1) were determined by real-time fluorescent quantitative PCR. Results: Lamina propria CD3CF lymphocyte, 5-HT-positive EC cell and CgA-positive EC cell counts in normal mucosa of patients with colonic adenoma were significantly higher than those in control group (P〈0.05). No significant difference of IEL count was observed between colonic adenoma and control groups (P=0.175). The mRNA expressions of TNF-a, COX-2, iNOS, TpH1 in normal mucosa in colonic adenoma group were significantly higher than those in control group (P〈0.05). The mRNA expression of TpH1 was positively correlated with mRNA expressions of TNF-a, COX-2 and iNOS (P〈0.05). Conclusions: Inflammation exists in normal mucosa of patients with colonic adenoma, and inflammation is correlated with the increased synthesis of 5-HT.
