摘  要:目的探讨声辐射力脉冲成像(ARFI)技术评价非酒精性脂肪性肝病(NAFLD)患者肝脏弹性的临床价值。方法分别对76例NAFLD患者和30例健康对照者应用声辐射力脉冲成像技术进行肝脏弹性的检测,比较并分析各组间肝脏弹性定量参数的差异。结果 NAFLD组与对照组间ARFI测值差异具有统计学意义(P<0.01),NAFLD不同程度组间ARFI测值差异具有统计学意义(P<0.01),重度与轻、中度组比较,差异具有统计学意义(P<0.01)。随NAFLD患者脂肪肝程度的加重,肝脏的ARFI测值也逐渐增高,两者存在正相关(r=0.453,P<0.01)。结论声辐射力脉冲成像技术不仅无创,更可较准确的对NAFLD患者肝脏弹性进行定量评价。Objective To investigate the clinical value of the acoustic radiation force impulse(ARFI) technique in the quantitative evaluation of non-alcoholic fatty liver disease.Methods 76 patients with non-alcoholic fatty liver disease and 30 normal healthy controls were examined using acoustic radiation force Impulse technique.Results The parameter of ARFI in non-alcoholic fatty liver disease group was significantly lower than that in the control group(P0.01).In non-alcoholic fatty liver disease patients,there were significant differences in the parameter of ARFI among the mild,moderate and severe fatty liver groups(P0.01).The difference in ARFI between the severe group and other two groups was significant(P0.01).A positive correlation was found between the stiffness of the liver and the sonography grade of fatty liver(r=0.453,P0.01).The ARFI parameter of the liver increased along with an increase in the degree of fatty liver.Conclusion Acoustic radiation force impulse technique not only can examine the stiffness of the liver noninvasively,but also has potential value for quantitative evaluation of the degree of non-alcoholic fatty liver disease.
