儿童先天性心脏病各分型致病因素的多因多果研究

摘　　要：目的 :为制定先天性心脏病一级预防措施提供科学的参考依据。方法 :对 5 10例先天性心脏病 ( CHD)患儿进行 1∶2病例对照配对调查 ,其中室间隔缺损、房间隔缺损、法乐氏综合征和动脉导管未闭 4种主要分型 CHD各 10 2例 ,其他分型CHD共 10 2例。在建立总的不分型 CHD与各危险因素的条件 L ogistic模型并筛选出 CHD总的危险因素之后 ,采用多个因变量和多个自变量的条件 L ogistic模型 ,对各分型 CHD与可疑危险因素进行多因多果分析 ,分别筛选出各分型 CHD相应的危险因素。结果 :不分型 CHD总的危险因素与各分型 CHD的危险因素不尽相同 ,各分型 CHD之间的危险因素也有差别。结论 :制定 CHD一级预防措施仅依据不分型总的病因研究结论是不全面的 ,还应考虑各分型Objective:To provide scientific basis for formulation of first grade of preventive measures for congenital heart diseases Methods:Altogether 510 cases of congenital heart diseases (CHD) were collected to carry out a case control study to probe into their causes according to the ratio of 1∶2,in which,there were 102 cases of ventricular septal defect,atrial septal defect,syndrome of Fallot,patent arterial duct and other type of CHD,respectively After all risk factors of CHD were picked out by conditional Logistic model with single dependent variable and multi independent variables,the same model with multi dependent variables and multi-independent variables was introduced to analyse the corresponding risk factors of various types of CHD Results:All risk factors of CHD and those of various types of CHD were not identical and there were also some differences among the various CHD Conclusion:The first grade of preventive measures formulated according to the all risk factors was incomprehensive and those of various types of CHD should be taken into consideration
