摘  要:目的检测血管紧张素Ⅱ1型受体(angiotensinⅡtype 1 receptor,AT1R)和血管紧张素转化酶(angiotensin converting en-zym e,ACE)在子宫内膜癌中的表达情况,探讨AT1R和ACE在子宫内膜癌患者预后判断和临床治疗等方面的价值。方法采用免疫组化EnV ision法染色,观察AT1R、ACE、血管内皮生长因子(vascu lar endothelial growth factor,VEGF)和微血管密度(m i-crovessel density,MVD)在18例正常子宫内膜、37例子宫内膜不典型增生和89例子宫内膜癌组织中的表达情况,分析AT1R和ACE与子宫内膜癌临床病理特征的关系,研究AT1R与VEGF和MVD的相关性。结果从正常子宫内膜上皮到子宫内膜不典型增生再到子宫内膜癌,AT1R的表达逐步上调,ACE的表达逐步下调,差异均有显著性。子宫内膜癌中AT1R的表达与患者年龄、临床分期、子宫肌壁浸润深度和淋巴结转移状况等有关,且AT1R的表达与VEGF的表达和MVD计数正相关,而ACE的表达仅与淋巴结转移状况有关。结论 AT1R可能参与子宫内膜癌的发生、发展,AT1R表达上调可促进癌细胞生长和新生血管形成,与子宫内膜癌的预后有关,AT1R可能成为治疗子宫内膜癌的新靶点。ACE在子宫内膜癌中的表达低于正常内膜组织,其具体机制有待于进一步研究。Purpose To study the expression of angiotensin Ⅱ type 1 receptor(AT1R) and angiotensin converting enzyme(ACE) in endometrial carcinoma and correlation with clinicopathologic variables,vascular endothelial growth factor(VEGF) and microvessel density(MVD),and to explore the value of AT1R and ACE in prognosis and treatment of endometrial carcinoma.Methods Immunohistochemical EnVision method was applied to detect expression of AT1R,ACE and VEGF in paraffin-embedded tissues from 18 cases of normal endometrial tissue,37 cases of atypical endometrial hyperplasia and 89 cases of endometrial carcinoma.The microvessel density(MVD) was assayed by counting CD31 positive blood vessels.Results From normal endometrium to endometrial hyperplasia and then to endometrial cancer,AT1R expression increased gradually and ACE expression gradually reduced,the differences were significant.AT1R expression in endometrial carcinoma was correlated with age,international federation of gynecology and obstetrics(FIGO) stage,depth of myometrial invasion and lymph node metastasis.Furthermore,AT1R expression was positively correlation with VEGF and MVD.Expression of ACE only related to lymph node metastasis.Conclusions Over-expression of AT1R may contribute to angiogenesis and progression of endometrial carcinoma and AT1R could be a novel,potential target for treatment of endometrial carcinoma.The positive expression rate of ACE in normal endometrial tissues was higher than that in endometrial carcinomas,this mechanism requires to be elucidated by further research.
