摘 要:目的:分析正畸力作用下局部骨重建差异,探讨增龄因素对微种植体支抗稳定性的影响。方法:以3月龄和6月龄雄性新西兰兔20只为实验动物,每组10只,在兔双侧上颌骨各植入微种植体1枚,共40枚。愈合3周后右侧加载,加载3周后处死,RT-PCR法检测微种植体周围骨组织骨保护素(OPG)及其配体(RANKL)mRNA的表达,比较幼年兔和成年兔微种植体支抗周围骨组织中表达的差异。结果:未加载的微种植体周围骨中OPG、RANKL mRNA表达3月龄组高于6月龄组。加载3周后的微种植体周围骨组织OPG mRNA表达均降低,3月龄组加载压力侧降低最明显;RANKL mRNA表达均升高,3月龄组加载压力侧升高最明显。加载后RANKL/OPG比值均升高,3月龄组压力侧升高最多。结论:3月龄组骨转换活跃,加载后微种植体周围尤其是压力侧骨吸收显著增强,可能导致微种植体支抗稳定性下降。6月龄组微种植体支抗周围骨重建相对平缓,有利于种植体的稳定。Objective: To analyze the discrepancy of bone remodeling aroud loaded microscrews and explore aging effects on the stability of orthodontic mieroserews by examining osteoprotegerin (OPG) and receptor activator of nuclear factor-KB ligand (RANKL) mRNA expression in pert-screw bones in juvenile and adult rabbits. Methods: Twenty male newzeland rabbils including ten 3-month old( young group) and ten 6-month old( old group) were used in the study. Forty self-drilling mieroscrews were implanted in maxilla of the rabbits on each side. After 3 weeks screws on right side were loaded by 150 g force. Rabbits were sacrificed at the test time. The level of OPG mRNA and RANKL mRNA in pert-screw bones was investigated with RT-PClt. Results: Significant higher level of OPG and RANKL mRNA around unloaded mieroserews was expressed in young group than that in old group. OPG mRNA around loaded mieroserews deereased and it decreased the most on the pressure side of the loaded screws in young group. RANKL mRNA around loaded microscrews increased and it increased the most on the pressure side of the loaded screws in young group. After loading, the RANKL/OPG in each group increased, which increased the most on the pressure side of the loaded screws in young group. Conclusion: With active bone turnover, the bone resorption around loaded microscrews in young group is strengthened especially on the pressure side, which indicates that such disruption in the balance between bone formation and bone resorption results in instability of orthodontie microscrews. However, inactive bone remodeling around loaded microscrews in old group is helpful for stability of orthodontic nlieroscrews.
