十二指肠恶性肿瘤的临床及X线表现(附12例分析)

摘　　要：目的：探讨十二指肠恶性肿瘤的临床特点及 Ｘ 线征象与诊断在外科手术中的价值。方法： 采用胃肠气、钡双重造影和低张双重造影方法，分析了１ ２ 例经病理证实的十二指肠恶性肿瘤的临床及 Ｘ 线资料。结果：１２ 例中， 腺癌９ 例、恶性淋巴瘤２ 例及平滑肌肉瘤１ 例；病变位于十二指肠球部１ 例、乳头部５ 例、乳头上区４ 例、乳头下区２ 例； Ｘ 线表现为不规则狭窄及充盈缺损，亦见不规则腔内钡斑。结论：十二指肠恶性肿瘤 Ｘ 线表现有一定的特点，若能结合临床并仔细观察 Ｘ 线征象， 可减少误、漏诊。某些 Ｘ线征象对于决定能否外科手术切除具有重要临床价值。Objective:To study the clinical appearances and the value of X-ray findings of malignant tumor of the duodenum on the operation of clinical surgery.Methods:The clinical data and radiological signs of gastrointestinal air-barium double series and low tension double series in 12 cases with duodenal tumor proved by pathology and surgery were analysed.Results:In 12 cases,9 cases were adenocarcinoma,1 case was primary malignant lymphoma and 2 cases were leiomyosarcoma.One case was in the duodenal bulb,5 cases were in the duodenal papilla pars,4cases were in the superior area of duodenal papilla pars and 2 cases were in the inferior area of duodenal papilla pars.Radiological findings included:irregular stricture,stiffeness,filling defect and irrgeular crater in the tract of the lesion.The clinical appearances were followed:obstructive jaundice or/and upper gastrointestinal obstruction.Conclusion:The duodenal malignant tumors are of certain characteristic radiological findings.The misdiagnosing rate will be reduced if we observe the radiological manifestations in detail and combined with the clinical features.Some X-ray findings of duodenal malignant tumor are of important clinical value for the operation of surgery treatment.
