肥胖儿童载脂蛋白E基因多态性的研究

摘　　要：目的　探讨单纯性肥胖儿童载脂蛋白Ｅ（ＡｐｏＥ） 基因多态性的分布及其对血脂、脂蛋白、载脂蛋白的影响。方法　采用改良的聚合酶链式反应限制性片段长度多态性方法分析肥胖及健康儿童ＡｐｏＥ 基因型。结果　肥胖儿童Ａｐｏ ε４ 等位基因频率（０．１２２ ９） 较健康儿童（０．０６１ ８） 增高（ Ｐ＜ ０．０５）。ＡｐｏＥ基因多态性影响健康儿童的血脂水平。在单纯性肥胖儿童，ε２ 、ε３ 、ε４ 等位基因携带者的血甘油三酯（ＴＧ） 、总胆固醇（ＴＣ）、低密度脂蛋白胆固醇（ＬＤＬＣ） 、ＡｐｏＥ 浓度有差异（ Ｐ＜ ０．０５） ；与ε３等位基因携带者比较，ε４ 具有较高的血ＴＧ、ＴＣ、ＬＤＬＣ水平和较低的ＡｐｏＥ 水平；ε２等位基因携带者的血ＴＣ、ＬＤＬＣ水平较低（ Ｐ＜ ０．０５）。ε２、ε３ 、ε４等位基因与血ＴＣ、ＬＤＬＣ浓度呈等级正相关，与血ＡｐｏＥ水平呈等级负相关（ Ｐ＜ ０．０５） ；且发现与健康儿童比较， 肥胖小儿ＡｐｏＥ等位基因对血ＬＤＬＣ、ＡｐｏＥ 浓度影响更明显（Ｐ＜ ０．０５）。结论　单纯性肥胖儿童有ＡｐｏＥ基因多态性的变化，且明显影响小儿血浆脂类代谢。Objective To investigate the distribution of apolipoprotein (apo) E polymorphism and association of apoE genotypes with plasma lipids in children with simple obesity Methods ApoE genotypes were determined by polymerase chain reaction restricted fragments length polymorphism Results Compared with control group, the frequency of apo ε 4 allele in obesity group was higher In obesity group, the levels of triglyceride, total cholesterol, low density lipoprotein cholesterol and apoE had significant differences among carriers of apo ε 2 , ε 3 and ε 4 alleles The children carrying apo ε 4 allele had higher levels of triglyceride, total cholesterol, low density lipoprotein cholesterol, lower levels of apoE than those carrying the other apoE alleles The children carrying apo ε 2 allele had lower levels of total cholesterol, low density lipoprotein cholesterol Conclusion Children with the simple obesity had different patterns of apoE polymorphisms as compared with healthy children ApoE polymorphisms may affect plasma lipids metabolism
