奥沙利铂联合卡培他滨与FOLFOX治疗晚期结直肠癌疗效比较

摘　　要：目的比较奥沙利铂联合卡培他滨与FOLFOX方案治疗晚期结肠、直肠癌的近期疗效和毒副反应。方法将64例经病理检查确诊的晚期结肠、直肠癌患者随机分为两组。治疗组采用奥沙利铂85 mg/m^2静脉滴注2 h,第1天;卡培他滨1000 mg/m^2口服,第1～14天;每3周为一个周期。对照组CF 200 mg/m^2静脉滴注2 h,第1～5天;5-Fu 300 mg/m^2静脉点滴5 h,第1～5天;每3周为一个周期。2个周期后按照WHO实体瘤近期客观疗效评定标准进行评价。结果治疗组与对照组的总有效率分别为62.5%(20/32)和34.4%(11/32),差异有统计学意义(χ~2=4.0,P<0.05);治疗组有2例患者CR;两组主要毒副反应为血液和神经毒性。结论奥沙利铂联合卡培他滨方案治疗晚期结肠、直肠癌近期疗效较FOLFOX方案好,虽然其恶心、呕吐、神经毒性较重,但患者能耐受。Objective To compare the efficacy and toxicities of L - OHP ＋ CAPE and FOLFOX regimens in the treatment of patients with carcinoma of coion and rectum Methods Sixty - four patients with pathologically diagnosed ad- vanced carcinoma of coion rectum were randomly divided into two groups. 32 patients in the treatment group were treated by L - OHP ＋ CAPE ,which included L - OHP 85mg/m2 intravenously in dl and CAPE 1000mg/m^2 PO from dl to d14. Another 32 cases in the control group were treated by FOLFOX regime including CF200mg/m^2 intravenously from dl to d5 and 5 - Fu 300 mg/m^2 intravenously from dl to dS. The treatment was repeated every 3 weeks. Efficiency was evaluated after two - cycle treatment was performed. Results The clinical characteristics of the two groups were similar （ P 〉 0.05 ）, All patients could be evaluated. The overall response rate was significantly different between L - OHP ＋ CAPE and FOLFOX regimen （X2 = 4. 0, P 〈 0. 005 ）, which was 62.5% （ 20/32 ） and 34. 4% （ 11/32 ） respectively. Two patients in the treatment group achieved CR. The major toxicities of the two groups included myelosuppession and neuronsensory toxicities. Conclsions L - OHP ＋ CAPE has higher response rate than FOLFOX regimen in the treatment of patients with d carcinoma of coion rectum. The toxicities is mild and well tolerated in both groups.
