CO_2激光联合Nd∶YAG激光治疗口腔颌面部淋巴管瘤

摘　　要：目的评估联合应用ＣＯ２激光和Ｎｄ∶ＹＡＧ激光治疗口腔颌面部淋巴管瘤的疗效。方法先以ＣＯ２激光汽化（切割）淋巴管瘤瘤体，然后将Ｎｄ∶ＹＡＧ激光光纤插入肿瘤基底进行照射。对６０例（男３４例，女２６例）术后达到或超过５年的患者（术后５年２５例，６～１０年２３例，＞１０年１２例）进行随访，并分析其临床资料。患者术时年龄１～５岁２８例，６～１０岁１８例，１１～１５岁１０例，１６～３０岁４例；毛细管型淋巴瘤３６例，海绵型３例，混合型２１例。结果术中激光照射时间３０～３００ｓ，仅２例有少量出血。手术创伤小，术后当天均能进食，无一例感染。创面愈合时间长于常规手术，但愈合良好，毛细管型者创面愈合后难以辨认原病损区。随访显示疗效优者３５例，良２２例，中３例，无一例无效与复发，总优良率达９５％；毛细管型疗效优于海绵型（Ｐ＜００１）和混合型（Ｐ＜００５）。结论提示两种激光联合治疗淋巴管瘤是一种有效的治疗手段。Abstract Objective There were many methods used to cure oral and facial lymphangiomas before, but many patients were relapsed. For the purpose for improving the curative effects, a clinical research on curing oral and facial lymphangiomas with CO2 laser combined with NdYAG laser has been carried out since 1981. Methods The superfacial lesions of lymphangiomas were vaporized by CO2 laser firstly and then, NdYAG laser irradiation was adopted to destroy the deeper part of the lesions after the optic fiber of the NdYAG laser was inserted into the lesions until the colour of the skin surface of the lymphangiomas became white. Results Sixty patients (male 34, female 26) were treated with this method and the irradiation time lasted 30 s to 5 min. All the patients were treated in clinic. During operation, 58 cases were no bleeding, 2 cases bled a little. 24 cases were sutured because of the area of lesion was big. The healing time was longer than usual operation, but no scar was left, especially in patients suffering from capillary type, it was even difficult to find the formal lesion areas. Over 5 years followingup study it was revealed that the effect on capillary type is better than that on cavernous and mixed types. The results of this group are as following: excellent is 58.3% (35 cases); good is 36.7% (22 cases) and poor is 5% (3 cases). Conclusions The combination of CO2 laser and NdYAG laser treatment for oral and facial lymphangiomas is a reliable and effective method.
