摘　　要：目的观察间隙连接蛋白43(Connexin43,Cx43)在人胃癌组织中的表达,并探讨其与胃癌的发生、发展和预后的关系。方法应用组织芯片技术和免疫组织化学方法检测1072例胃癌组织中Cx43的表达,并分析其与癌浸润程度、分化、分期等病理特征及预后之间的相关性。结果 Cx43在T1、T2期和T3、T4期胃癌组织中的阳性表达率分别为51.6%和41.3%(P=0.002);在高/中分化和低/未分化的胃癌组织中阳性表达率分别为48.1%和38.8%(P=0.003);≤6cm癌组织中的Cx43阳性表达率为46.2%,>6cm的癌组织中为38.1%(P=0.024);Cx43在无淋巴结转移和有淋巴结转移的胃癌组织中阳性表达率分别为50.3%和41.3%(P=0.006);Cx43在Ⅰ、Ⅱ期和Ⅲ、Ⅳ期胃癌组织中的阳性表达率分别为50.1%和40.5%(P=0.002)。生存分析显示,Cx43是胃癌患者的独立预后因素。结论 Cx43的表达与人胃癌的组织分化、浸润程度、淋巴结转移、TNM分期和预后相关,可能参与人胃癌发生发展的过程,并可能是胃癌预后相关的指标之一。Objective To investigate the expression of connexin43（Cx43） in human gastric cancer and its clinical significance.Methods One thousand and seventy-two human gastric cancer tissues were detected by immunohistochemistry and tissue microarray for Cx43 expression.The relationship between Cx43 expression and clinicopathological factors was analyzed.Results The positive rates of Cx43 expression in T1/T2 and T3/T4 gastric cancer tissues were 51.6% and 41.3%,respectively（P=0.002）.The positive rates of Cx43 expression in well/moderately differentiated and poorly-differentiated/indifferentiated gastric cancer tissues were 48.1% and 38.8%,respectively（P=0.003）.The positive rates of Cx43 expression in gastric cancer tissues below 6 cm in diameter and over 6 cm in diameter were 46.2%and 38.1%,respectively（P=0.024）.The positive rates of Cx43 expression in negative lymph node metastasis and positive lymph node metastasis gastric cancer tissues were 50.3% and 41.3%,respectively（P=0.006）.The positive rates of Cx43 expression in stage Ⅰ/Ⅱ and Ⅲ/Ⅳ gastric cancer tissues were 50.1% and 40.5%,respectively（P=0.002）.According to the Cox proportional hazards model,Cx43 expression was an independent prognosis factor.Conclusion The expression of Cx43 is closely related with well differentiation,invasion,status of lymph node,clinical stage and prognosis.It may participate in the development and progression of gastric cancer,and may be used to predict the prognosis of gastric cancer patients.
